Traditional approaches to rehabilitation of disabled people have emphasised segregated institutio... more Traditional approaches to rehabilitation of disabled people have emphasised segregated institutional care, and have largely neglected the need both to integrate people with disabilities into their wider community, and for community attitudes themselves to be rehabilitated or changed to facilitate this integration. This paper reviews institutional and community gased strategies for rehabilitation, arguing the case for a community orientation but recognising some of the difficulties of implementing this effectively in developing countries.

However, it is unknown whether this attentional bias is specifically related to delusions or more generally to psychosis since the absence of the bias has not been established in people with psychosis (without delusions). It is also unknown when this bias develops in the course of psychotic disorder. These issues are investigated in people with first episode psychosis (FEP). The Stroop task was administered to three participant groups: a FEP group with current delusions (n = 31), a FEP group without current delusions (n = 11), and a non-psychiatric control group (n = 25). The FEP with delusions group showed a selective attention to threat words compared to the other groups (p = .02). Selective attention to threat is specifically related to delusions, is present early in the course of psychosis and may be candidate for new cognitive-behavioural therapies to treat medication-resistant delusions.

We assessed the concurrent validity of the Hospital Anxiety and Depression Scale (HADS) and the G... more We assessed the concurrent validity of the Hospital Anxiety and Depression Scale (HADS) and the Geriatric Depression Scale (GDS) against the Hamilton Rating Scale for Depression (Ham-D) in patients with Parkinson’ disease (PD). Forty-six non-demented PD patients were assessed by a neurologist on the Ham-D. Patients also completed four mood rating scales: the HADS, the GDS, the VAS and the Face Scale. For the HADS and the GDS, Receiver Operating Characteristics (ROC) curves were obtained and the positive and negative predictive values (PPV, NPV) were calculated for different cut-off scores. Maximum discrimination between depressed and non-depressed PD patients was reached at a cut-off score of 10/11 for both the HADS and the GDS. At the same cut-off score of 10/11 for both the HADS and the GDS, the high sensitivity and NPV make these scales appropriate screening instruments for depression in PD. The self-regulatory appraisals of 25 socially anxious and 25 nonanxious men were compared using a modified version of the Articulated Thoughts in Simulated Situations (ATSS) procedure (Davison, Robins, & Johnson, 1983). Subjects viewed videotapes of two enacted situations between a male actor and a female actor which differed on whether the individuals were strangers or acquaintances. The subjects were instructed to identify with different male actors who depicted contrasting styles of self-presentation. In contrast to nonanxious men, socially anxious subjects consistently made pessimistic self-appraisals, articulating more negative self-focused thoughts, and displaying negative self-other biases. Individual placement and support (IPS) may be effective at assisting the vocational recoveries of young people with first-episode psychosis (FEP).AimsTo examine the effectiveness of IPS at assisting young people with FEP to gain employment (Australian and Clinical Trials Registry ACTRN12608000094370). Young people with FEP (n = 146) who were interested in vocational recovery were randomised using computer-generated random permuted blocks on a 1:1 ratio to: (a) 6 months of IPS in addition to treatment as usual (TAU) or (b) TAU alone. Assessments were conducted at baseline, 6 months (end of intervention), 12 months and 18 months post-baseline by research assistants who were masked to the treatment allocations. Objective: This study explored auditory verbal hallucinations (AVH) and other psychotic symptoms ... more Objective: This study explored auditory verbal hallucinations (AVH) and other psychotic symptoms in youth with borderline personality disorder (BPD). Methods: Sixty-eight outpatients, aged 15-25 years, were categorized into three groups according to their primary DSM-5 diagnosis and AVH symptom profile; BPD+AVH (n=23), schizophrenia spectrum disorder (SZ)+AVH (n=22), BPD with no AVH (n=23). Results: No differences in AVH were found between BPD+AVH and SZ+AVH. Compared with SZ+AVH, BPD+AVH scored lower on delusions and difficulty in abstract thinking, and higher on hostility. BPD+AVH reported more severe self-harm, paranoid ideation, dissociation, anxiety, and stress than BPD no AVH. Conclusions: This study replicates in a sample of youth the finding from studies of adults that AVH in BPD are indistinguishable from those in SZ, when assessed by the Psychotic Symptom Rating Scales (PSYRATS). Clinicians should enquire whether youth with BPD experience AVH and consider them an indicator of a more severe form of BPD.

There is a lack of consensus regarding what it means to be socially included and what the key contributors to social inclusion may be. The aim of this investigation was to determine such key contributors, as identified by those with professional experience. A thematic analysis of literature regarding social inclusion was conducted to obtain the opinions of professionals regarding key contributors of social inclusion. Seventy-one pieces of literature were reviewed: peer-reviewed literature (academic literature regarding social inclusion in general [ = 25] and social inclusion and mental illness [ = 26]), and gray literature (organizational reports [ = 20]). However, the clinical significance of subthreshold features of BPD has not been investigated among real-world patients during the clinical emergence of the disorder, which is usually between puberty and emerging adulthood. This study aimed to replicate and extend previous research by comparing outpatient youth aged 15-25 years with subthreshold BPD features with youth with no BPD features. The sample included 499 potential participants, of whom 111 had no DSM-IV BPD features at all, and 155 had between one and four features. Affective and cognitive theory of mind abilities in youth with borderline personality disorder or major depressive disorder

Theory of mind (ToM) is an important social cognitive ability that has been investigated in BPD, ... more Theory of mind (ToM) is an important social cognitive ability that has been investigated in BPD, with inconsistent findings indicating impaired, comparable, and enhanced ToM in BPD. This study aimed to clarify and extend previous findings by investigating affective and cognitive ToM abilities in youth early in the course of BPD, by including a clinical comparison group of youth with major depressive disorder (MDD). Female participants aged 15-24 years diagnosed with BPD (n = 41) or MDD (n = 37) completed the Reading the Mind in the Eyes Test (RMET) and Happé&amp;amp;amp;amp;#39;s Cartoon Task, measures of affective and cognitive dimensions of ToM, respectively. The BPD group performed significantly worse than the MDD group on the affective ToM task, even after controlling for age, intelligence and depressive symptoms. Results for cognitive ToM were not significantly different. Finding of poorer performance on a measure of affective ToM, in BPD youth, relative to youth with MDD early in the course of BPD suggest a developmental failure of sociocognitive abilities needed for mentalising and which are theorised as giving rise to core features of BPD. Future research should employ more naturalistic paradigms to study social cognition and should assess individuals even earlier in the course of BPD.

We argue that a diagnostic formulation should be made prior to behavioural assessment but consider diagnosis as complementary to traditional behavioural assessment. Implicit in our proposal, is the notion that behavioural practitioners should be thoroughly conversant with the various forms of psychopathology, and be able to identify the signs and symptoms that cluster together to constitute a clinical syndrome or disorder. The same practitioners should be able to differentiate the signs and symptoms of one syndrome or disorder from another. It is further proposed that behavioural practitioners need to be aware of the limitations and boundaries of their particular approaches and concede the value of other treatment approaches (especially chemotherapy) derived from different theoretical perspectives (Hersen 1979, 1981; Kazdin and Hersen, 1980).
