There is a growing interest in whether and how to adapt psychotherapies to take into account the ... more There is a growing interest in whether and how to adapt psychotherapies to take into account the cultural, linguistic, and socioeconomic context of diverse ethnocultural groups. At the root of the debate is the issue of whether evidence-based treatments (EBTs) developed within a particular linguistic and cultural context are appropriate for ethnocultural groups that do not share the same language, cultural values, or both. There is considerable evidence that culture and context influence almost every aspect of the diagnostic and treatment process. Yet, there are concerns about fidelity of interventions, and some have questioned whether tinkering with well-established EBTs is warranted. We present arguments in favor of the cultural compatibility and universalistic hypotheses. Next, we review the available published frameworks for cultural adaptations of EBTs and offer examples from the literature on the process and outcome of different approaches used. Conceptual models for adapting existing interventions and emerging evidence that adapted intervention leads to positive outcomes suggest that there are tools for engaging in evidence-based psychological practices with ethnocultural youth. Recommendations for future directions are provided.

Its prognostic implication in prostate cancer (PCa) is unclear. We analyzed the impact of MS in the survival of men with PCa treated surgically. We studied patients with PCa, treated surgically between 1990 and 2007, and compared the survival of men with MS (group 1) and without MS (group 2). A subgroup analysis of those in stage pT2 was also performed. We calculated biochemical progression-free survival (bPFS) and cancer-specific survival, and the relation of clinical and pathological variables with these end-points. 65 men had MS. The 5- and 10-year bPFS in group 1 was 36 and 32% vs. 72 and 68% in group 2 (p < 0.0001). In multivariate analysis, prostate-specific antigen (p = 0.001) and MS (p < 0.0001) predicted biochemical progression/recurrence (BP/R). There was no difference in cancer-specific survival between groups (p = 0.40). Of 146 men in stage pT2, 38 had MS; group 1 men had worse 5- and 10-year bPFS (55 and 48%) than group 2 (80 and 73%; p = 0.001). In multivariate analysis, MS was the strongest predictor of BP/R (p = 0.0007). MS is related to adverse characteristics in PCa and confers poor bPFS after radical prostatectomy. This social phenomenon is associated with stress, depression, and mental health problems. This instrumental study reports on the development and evaluation of a Social Support Scale (CAS, for its name in Spanish), as part of a larger study that aims to develop valid and reliable psychosocial instruments in the Puerto Rican context. The CAS was designed to measure the need for emotional, interpersonal and material support. Also, satisfaction with the support received was measured. Four hundred and sixty-four Puerto Rican students from the University of Puerto Rico participated in this study. This paper examines attrition among applicants to a methadone maintenance program before and afte... more This paper examines attrition among applicants to a methadone maintenance program before and after it began to require that applicants have a relative or close friend willing to act as a treatment sponsor. Few applicants were unable to locate sponsors. The dropout rate was greater for Blacks than for Hispanics and Anglo Whites both before and after the sponsor requirement was introduced. However, young addicts, addicts with recent drug histories, and singles were somewhat less likely to enter treatment after the requirement was introduced. Implications for drug treatment clinicians and administrators are discussed.

Se evaluó la sintomatología depresiva, la ideación suicida, así como la presencia o historia de experiencias atípicas, eventos traumáticos y maltrato o abuso. Las experiencias atípicas estuvieron asociadas (p<.05) a ser fémina, a tener enfermedades físicas, a experimentar eventos traumáticos, a haber recibido tratamiento para la depresión, y a una sintomatología depresiva e ideación suicida elevada. Los eventos traumáticos, el tratamiento farmacológico para la depresión y la severidad de la ideación suicida fueron predictores significativos del número de experiencias reportadas. Los resultados confirman que la mayoría de las experiencias no son psicóticas. Se discuten las implicaciones para la evaluación y el tratamiento de jóvenes deprimidos/as.

Our objective was to evaluate the sensitivity, specificity, predictive value, and accuracy of com... more Our objective was to evaluate the sensitivity, specificity, predictive value, and accuracy of computed tomographic urography for the detection of bladder tumors in patients with microscopic hematuria. Patients with microscopic hematuria initially evaluated with computed tomography and cystoscopy from January 2006 to December 2009 were evaluated. Computed tomography detecting a bladder lesion suspicious of malignancy was considered positive. Cystoscopy was classified as positive when a lesion requiring biopsy or resection was found. Performance characteristics of computed tomography were determined by comparing with cystoscopic and pathological findings. A total of 112 patients were eligible for analysis. Seven tumors were found on cystoscopy; of these, 2 were correctly diagnosed by computed tomography and 5 were missed. An additional case was considered erroneously positive. The results are a sensitivity of 29%, specificity of 99%, positive predictive value of 67%, negative predictive value of 95%, and accuracy of 95%. Although computed tomography has a high specificity its sensitivity is limited. For this reason conventional cystoscopy should be considered the standard for bladder evaluation of patients with microscopic hematuria.
