A small novelty effect with age was found in men for daytime SBP, increasing from 11.7-23.1% (P = .03, chi-square test). In women, it was constantly above 20% (Table 1). In conclusion, 7-day/24-hour ABPM reveals a statistically significant pressor response in women and in elderly men, reflecting a novelty response to the monitor. Day-today variation in SBP was observed during the daytime but not the nighttime. Whereas clear advantages of ABPM over office BP measurements have been established to assess the efficacy of antihypertensive medication, the present results indicate that ABPM is useful for a more-precise diagnosis that accounts for a novelty effect or for a weekly variation, leading to better treatment, especially for elderly adults.

They also represent more than two-thirds of adverse events in hospitals, mainly affecting patients older than 65 years. Physical activity is a central intervention in fall prevention for older people. Whatever the details of the prevention strategy that is adopted (ie, how a mono- or multifactorial intervention is evaluated, the category of person the intervention targets, and where it is used), it is important to ensure that the proposed intervention is feasible and usable for the patient and the health care team. Objective The primary objective is to study the usability of carrying out a physical activity intervention, including 3 types of exercises, in older patients hospitalized in a geriatric acute care unit and categorized according to 3 fall risk levels: low, moderate, and high. The secondary objectives are to determine the difficulty of the physical exercise for pat...

Gerontechnology represents an asset in the face of this challenge. Surprisingly, the hospital environment is not very well equipped. This is why the university hospital center of Angers has created the ALLEGRO living lab in order to promote the development of gerontechnology in the health sector with the active participation of users.

Research in LLH (Living labs in health) covers a wide range of studies from non-interventional studies to CT (clinical trials) and should involve patients with neurocognitive disorders. However, the ethical issues raised by the design, development, and implementation of research and development projects in LLH have been the subject of only little interest thus far. Objective Our aim was to determine a pragmatic, ethical and regulatory correct approach to seek the informed consent of patients with neurocognitive disorders according to the different types of studies carried out in European LLH, with a focus on the French context. Methods A narrative review of regulatory texts and clinical articles was conducted, and a pragmatic procedure to determine the decision-making capacity of older adults in LLH was proposed. Results Individuals must ...

The Ardoiz® tablet has been designed with a simplified interface for older adults. OBJECTIVE To assess the perceived usability and satisfaction of the Ardoiz® tablet. METHODS We conducted a mixed methods with cross-sectional study using System Usability Scale (SUS), satisfaction score and workshops, including geriatric patients, healthcare professional and caregivers. RESULTS Between September 25, 2019 and March 11, 2020, 58 participants were included in a cross-sectional study (including 38 patients, mean ±SD 85±6 years, 66% women), 26 in workshops (including 5 patients, mean ±SD 86.4±2.9, 40% women). The SUS was 74±12/100, the satisfaction score was 2.8±0.9/4, with 59% of satisfied participants with the use of Ardoiz® pads. The intent to acquire remained low with 18% (n=6) of participants who would be interested in acquiring the tablet. This tablet computer seemed to be difficult to use by ge...

OBJECTIVE Our objectives were to determine i) which virtual communication modality (i.e. phone-call versus video-call) was preferred by confined older patients and residents, and ii) the variables influencing this choice. METHODS Cross-sectional study to examine the preference between phone-calls and video-calls among frail older adults either hospitalized in a geriatric acute care unit (GACU) or institutionalized in a long-term care and nursing-home (LTC/NH) during COVID-19 confinement. RESULTS A total of 132 seniors were included between March 25 and May 11, 2020 (mean±SD, 88.2±6.2 years; 59% women). Patients hospitalized in GACU were more often independent to establish communication than residents institutionalized in LTC/NH (p=0.03) and were more satisfied (p=0.02). Overall, seniors tended to favor phone-c...

To promote gerontechnology innovation in the hospital and bridge the gap between gerontechnology developers and hospitalized frail older patients, our objective was to create and implement a hospital-based geriatric living lab. We designed a hospital-based living lab, providing reflexive workshops bringing around the table gerontechnology users and developers, supplemented with an experimental hospital room receiving both the users and the devices to be tested. Three different types of users were distinguished: seriously ill older inpatients, professional hospital caregivers, and informal carers. Three different kinds of devices were also distinguished: prototypes under development, new services and/or care organizations, and new uses. Finally, we were able to open in 2018 the Angers Living Lab En GéRiatrie hOspitalière (ALLEGRO) hospital-based geriatric living lab. ALLEGRO offers the organization of "idea incubator workshops" for users and developers, together with one "experimental hospital room" equipped with validated devices to provide reference measures used as a standard to test the diagnostic efficacy of prototypes. The room is intended to accommodate one older inpatient with severe acute organic failures. No patient selection is planned at admission, apart from consent to research. Until now, no refusal to participate in a study was noted. In conclusion, we offer a new and unprecedented hospital-based geriatric living lab to improve hospital care for older inpatients and to promote successful aging through gerontechnology.

Living labs are an open innovation research method for the development of new products and services. ALLEGRO is a hospital-based geriatric living lab designed to promote open gerontechnology innovation within hospitals. ALLEGRO bridges the gap between gerontechnology developers and hospitalized frail elderly people and carers. Gerontechnology developed in ALLEGRO should improve hospital care for older inpatients.

This study aims to examine performance criteria (i.e.; sensitivity, specificity, positive predictive value, negative predictive value and accuracy) for fall prediction resulting from a nurse assessment and an artificial neural networks (ANNs) analysis in older inpatients hospitalized in acute care medical wards. Methods: A total of 848 older inpatients (mean age, 83.0±7.2 years; 41.8% female) admitted to acute care medical wards in Angers University hospital (France) were included in this study using an observational prospective cohort design. Within 24 hours after admission of older inpatients, nurses performed a bedside clinical assessment. Participants were separated into non-fallers and fallers (i.e.; ≥1 fall during hospitalization stay). The analysis was conducted using three feed forward ANNs (multilayer perceptron [MLP], averaged neural network, and neuroevolution of augmenting topologies [NEAT]). Results: Seventy-three (8.6%) participants fell at least once during their hospital stay. ANNs showed a high specificity, regardless of which ANN was used, and the highest value reported was with MLP (99.8%). In contrast, sensitivity was lower, with values ranging between 98.4 to 14.8%. MLP had the highest accuracy (99.7). Conclusions: Performance criteria for fall prediction resulting from a bedside nursing assessment and an ANNs analysis was associated with a high specificity but a low sensitivity, suggesting that this combined approach should be used more as a diagnostic test than a screening test when considering older inpatients in acute care medical ward.
