Rotational evaluation of humeral shaft fractures with proximal extension fixed using the MIPO technique

OBJECTIVE Our objective was to evaluate possible rotational deviations using minimally invasive plate osteosynthesis (MIPO) and to determine how it affects upper limb function. METHODS We followed 11 patients for a period of two years. The proximal approach was anterior-lateral, and the distal approach was anterior, dividing the brachial muscle. For rotational evaluation, we used the semi-axial incidence described by Oztuna et al., assessing the degree of retroversion of the humeral head in relation to the elbow. During follow-up, we measured bilateral range of motion, shoulder function (UCLA), and upper limb function (DASH). RESULTS For patients whose final result was an increase in retroversion, there was an average difference of 9°, with an increase in the final difference of external rotation of 3° compared with the other shoulder, and no difference between the final levels of internal rotation. In the patients whose final result was a decrease in retroversion, where the distal fragment was fixed in internal rotation, the difference observed was an average of 6°. These patients had an average decrease of the external rotation of 5° with an increase of the internal rotation by two levels. The final functional scores were "good" and "excellent" in all patients, with a mean UCLA of 31.8 points (28-34) and a mean DASH of 9.11 points (0.83-22.2). CONCLUSION In all patients, there was a difference in the humeral head retroversion compared to the contralateral limb, but with little clinical repercussion and good or excellent functional scores.

Scripting languages are easy to learn and use, and they allow complex tasks to be specified and executed in fewer steps than with traditional programming languages. However, they also have important limitations for reproducibility and data management. As experiments are iteratively refined, it is challenging to reason about each experiment run (or trial), to keep track of the association between trials and experiment instances as well as the differences across trials, and to connect results to specific input data and parameters. Approaches have been proposed that address these limitations by collecting, managing, and analyzing the provenance of scripts. In this article, we survey the state of the art in provenance for scripts. We have identified the approaches by following an exhaustive protocol of forward and backward literature snowballing. Based on a detailed study, we propose a taxonomy and classify the approaches...

A 76-year-old woman was referred to our service for dysphagia, blockages, and retrosternal pain. She experienced several episodes of aspiration pneumonia. The esophagogram showed a large diverticulum, 10 cm above the diaphragm, with an important contrast reflux. A manometry was performed and showed no aperistalsis but the esophagogastric junction could not be passed. We performed a computed tomography (CT) scan with oral contrast, which showed a 3.5 × 5.7 cm epiphrenic diverticulum (▶ Fig. 1). Thoracic surgery was contraindicated for this elderly woman and peroral endoscopic myotomy (POEM) [2-4] was proposed to cut the septum. We performed a submucosal tunneling dissection 7 cm above the diverticulum using a 1.5 mm Dual Knife (Olympus, Tokyo, Japan). Once we reached the septum, we dissected the submucosa until we reached the periesophageal space in the mediastinum. Then, the tunnel was continued 5 cm under the diverticulum in the esophageal axis to expose the septum. Myotomy was performed selectively on the circular fibers (▶ Fig. 2, ▶ Video 1). The tunnel entrance was then closed using five hemostatic clips (Boston Scientific, Marlborough, Massachusetts, USA). The procedure was performed without any complications with the patient under general anesthesia. Postoperative contrast swallow showed no difference from the preoperative test (▶ Fig. 1 b). The patient resumed feeding at postoperative Day 1 and was discharged at Day 3. At follow-up 2 months later, she was asymptomatic without any aspiration or dysphagia. ▶ Fig. 1 The epiphrenic diverticulum. a Contrast swallow showed a large epiphrenic diverticulum in the lower esophagus with no clear stenosis. b Contrast swallow after peroral endoscopic myotomy (POEM). c Computed tomography scan before POEM (AL, anterior left; PR, posterior right). d Endoscopic aspect of the septum before POEM. During the demo, we will show how noWorkflow collects and manages provenance, as well as how it supports the analysis of computational experiments. We will also encourage attendees to use noWorkflow for their own scripts.

The galactose-induced decrease in phosphate levels leads to toxicity in yeast models of galactosemia
Biochimica et biophysica acta, Jun 14, 2017

A number of evidences indicate that the galactose-1-phosphate accumulation observed in patient cells is a cause of toxicity in this disease. Nevertheless, the consequent molecular events caused by the galactose-1-phosphate accumulation remain elusive. Here we show that intracellular inorganic phosphate levels decreased when yeast models of classic galactosemia were exposed to galactose. The decrease in phosphate levels is probably due to the trapping of phosphate in the accumulated galactose-1-phosphate since the deletion of the galactokinase encoding gene GAL1 suppressed this phenotype. Galactose-induced phosphate depletion caused an increase in glycogen content, an expected result since glycogen breakdown by the enzyme glycogen phosphorylase is dependent on inorganic phosphate. Accordingly, an increase in intracellular phosphate levels suppressed the galactose effect on glycogen...
