Aim: The aim of the study was to evaluate endothelial dysfunction in systemic vascular territory ... more Aim: The aim of the study was to evaluate endothelial dysfunction in systemic vascular territory in a group of patients with cirrhosis divided into subgroups according to etiology and ARPI score, compared with a control group. Method: We conducted a study including 78 patients with cirrhosis in which endothelial function at rest and at effort was determined using an accurate parameter in estimating endothelial dysfunction-single passive limb movement-sPLM. Results: At rest, after comparing the main two groups, no statistically significant differences were noted, yet in the comparison of the cirrhotic patients subgroup with ethanoic etiology and the control group, a significant statistically difference was observed. Conclusions: The hypothesis that patients with cirrhosis are showing increased NO-mediated systemic vasodilatation was confirmed in our study, only after performing a vasodilating maneuver such as single passive limb movement. Interestingly, additional vasodilating effects was observed at rest, especially after sPLM in the subgroup of patients with ethanolic cirrhosis, but also in the subgroup with APRI score less than 1 at rest and over 1.5 after sPLM by mechanisms that require a better understanding. Rezumat: Scop: Scopul studiului a fost evaluarea funcției endoteliale în teritoriul vascular sistemic la un grup de pacienți cu ciroză hepatică împărțiți în subgrupuri în funcție de etiologie și scorul APRI comparativ cu un lot martor. Metodă: A fost efectuat un studiu pe 78 de pacienți cu ciroza hepatica la care s-a făcut determinarea funcției endoteliale la repaus și la efort utilizând un parametru dovedit a fi precis ca și estimare a funcției endoteliului-mișcarea pasivă a unui membru inferior (single passive limb movement-sPLM). Rezultate: La repaus, după compararea celor două grupuri principale, nu au fost remarcate diferențe semnificative statistic, dar la compararea subgrupului de pacienți cirotici etanolici cu grupul de control, a fost observată o diferență semnificativă statistic. Concluzii: Ipoteza că pacienții cu ciroză hepatică manifestă o vasodilatație sistemică crescută NOmediată a fost confirmată în studiul nostru, însă doar după o manevră vasodilatatoare ca mișcarea pasivă a unui membru inferior. În mod interesant un efect suplimentar vasodilatator a fost observat și la repaus, mai ales după sPLM la subgrupul de pacienți cu ciroză hepatică etanolică dar și la subgrupul cu scor APRI mai mic de 1 la repaus și peste 1.5 după sPLM, prin mecanisme ce necesită o mai bună înțelegere.

Objective: Identification and differentiation of community acquired and non-community acquired pneumoniain order to identify and treat early possible complications such as reactivation of pulmonary tuberculosis. Discussions: The particularity of the case is that we have a patient without knownimmunodeficiencies, who develops non-community acquired pneumonia with Klebsiella Pneumoniae, with the reactivation of tuberculosis. The pathology caused by bacteria belonging to the genus Clostridium is a major cause of economic ... more The pathology caused by bacteria belonging to the genus Clostridium is a major cause of economic loss in small ruminant herds. The clinical manifestations caused by these pathogens are the result of a wide and variable spectrum of toxins released during their multiplication. The aim of this study was to enhance the toxigenesis of one of the Clostridium septicum isolates and to select the most productive clone, being the closest to the field strain. The strain toxicity was improved by passages on guinea pigs. In vivo assays on balb/C mice were confirmed by in vitro experiments (Western Blott, semi quantitative dosing of cytopathic effect and quantification of hemolytic activity). Assessment of the immunizing value was carried out by seroneutralization in mice. The selected and improved strain produced a toxin with a biological value superior to the minimum recommended limits for the formulation of anaerobic vaccines for small ruminants.
