Y perforator flaps for facial reconstruction have gained great popularity in the last years. The advantages compared to randomized local flaps are the marked increased arc of rotation and transfer length as well as less lymphatic congestion. The paper aims to show the field of application in facial reconstruction and has the purpose to serve as a continuing education. Methods. With 6 clinical case presentations the versatility and some refinements of V/Y perforator flaps for facial reconstruction are presented. The general principle of the operative procedure is explained in detail. Results. Beside the well-known treatment options for the different facial areas with different perforator containing layers a novel myocutaneous forehead flap is introduced for reconstruction of a supraorbital defect. Conclusion. During the last years, V/Y perforator flaps have become an important tool for functional and aesthetical sufficient facial reconstruction. And it has proven its right of existence in facial plastic surgery.

The main body of the flap is advanced to cover the external surface of the perialar and alar defects. A side extension to the flap, based only on a dermal blood supply, is turned over, to line the reconstructed alar rim.

Jahrhunderts auch die Plastische Chirurgie. Insbesondere ging es dabei um die Facelift-Chirurgie und die beiden deutschen Pioniere in diesem Feld: Eugen Holländer und Erich Lexer. Ziel der Arbeit Bei der Entwicklung der Facelift-Techniken wirkten die beiden Chirurgen Eugen Holländer und Erich Lexer entscheidend mit. Die Arbeit untersucht wie sich diese beiden Chirurgen mit dem Thema beschäftigten und welche Erkenntnisse sich daraus für die weitere Entwicklung der Facelift-Chirurgie ergaben. The ala of the nose is vascularized by a dense net of perforators originating from the lateral na... more The ala of the nose is vascularized by a dense net of perforators originating from the lateral nasal artery or the angular artery. These vessels reach the ala in a cascade fashion from the alar groove/lateral nasal wall. Based on these vessels, a V/Y flap can be dissected with a wide range of mobility. Nineteen patients underwent reconstruction of the nose by means of a V/Y-alar-perforator flap alone or in combination with a myocutaneous rotation flap from the lateral nasal wall and/or from the nasal dorsum for reconstruction of defects at the nasal tip, ala of the nose, and/or lateral nasal wall. All patients were followed up 8 months after surgery in average. There was no prolonged congestion or any tissue loss. All patients presented postoperatively with excellent aesthetic results, and no nasal distortion was observed. In particular a main interest was hereby the so-called facelift surgery and the first two German pioneers in this field: Eugen Holländer and Erich Lexer. AIM OF THE PAPER The surgeons Eugen Holländer and Erich Lexer contributed to a big part to the development of facelift techniques. The paper aims to highlight in which way both surgeons conducted their research work and the topic of facelift and which findings could be drawn out of their work. Nasolabial flaps are commonly used. With these techniques nasolabial and perialar skin is subdermally dissected and turned over into the defect to create both the inner and outer lining of the ala. If there is also loss of the perialar and nasolabial skin, such techniques become impossible. With a modification it is still possible to reconstruct both the ala of the nose and the perialar/nasolabial skin in one operation. A V/Y sliding flap from the cheek with a side extension from the lower nasolabial fold used as a turn-over flap is dissected. With this technique it is also possible to reconstruct isolated loss of the ala of the nose.

The literature suggests that Dieffenbach or von Langenbeck or von Esmarch was the first to introduce this procedure. A systematic review of the German literature from the 19th century revealed that it was von Bruns who performed the vermilionectomy for the management of lip malignancies for the first time.

A huge number of procedures for forehead and brow rejuvenation have been described. Nevertheless,... more A huge number of procedures for forehead and brow rejuvenation have been described. Nevertheless, the surgical approach of brow aesthetics in terms of correction of brow fullness and symmetry has not been systematically evaluated in the literature. We recently proposed a 20-degree beveled brow incision technique for direct brow lifting. The aim of the present study is to evaluate the 20-degree beveled brow incision technique for brow reduction, reshaping and correction of brow symmetry. Eighteen patients underwent brow reshaping by using the 20-degree beveled brow incision technique combined or not with other lifting procedure (brow lift, blepharoplasty, face lift). The surgical outcome was evaluated by photographic documentation and a questionnaire on patient satisfaction. Follow-up was at least 6 months. The surgical outcome in terms of brow symmetry, fullness and scaring was very good in 14 of 18 patients, good in 2 patients and poor in 2 patients. The last two patients were smokers. Motor or sensibility disorders were not observed. The 20-degree beveled brow incision technique seems to be saved and effective as surgical procedure to improve brow aesthetics in terms of symmetry and fullness while a supplement lifting maneuver is easily to perform.

The two radioiodinated fatty acid analogs 15-(para-131l-phenylpentadecanoic acid (pPPA) and 15-(o... more The two radioiodinated fatty acid analogs 15-(para-131l-phenylpentadecanoic acid (pPPA) and 15-(ortho-131l-phenyl)-pentadecanoic acid (oPPA) are isomers with individually different routes in lipid metabolism but with near equal transport kinetics into tissue. Methods: Normal adult male Wistar rats (n = 79) and those with liver cell damage from adriamycin treatment (n = 84) received 1.48-1.85 MBq 131l-pPPA or 131l-oPPA (specific activity, 33.3-46.3 GBq/fJW) into the jugular vein. At 1,2,3,5,7,10 and 20 min, livers of up to five animals per group were examined for total tracer uptake and tracer incorporation into various lipid fractions. Results: Uptake of both isomers into the total liver plateaued at about 2 min; the ratio oPPA/pPPA in normal liver averaged 2.63 and was significantly higher than the average ratio of 1.50 after adriamycin treatment. This fall in ratio was mainly due to an increase of pPPA uptake. Significant differences of the respective ratios were found in the plateau for the phospholipids (9.7 versus 3.0), cholesterol (2.4 versus 0.7) and triglycérides(2.0 versus 0.4). Conclusion: The dual-tracer technique with pPPA and oPPA promises to be clinically useful for the diagnosis of liver disease by imaging the ratios of tracer uptake in the total liver and by in vitro analysis of the uptake ratio in serum triglycérides.
