We report the use of a custom made fenestrated endograft in two patients with aortic arch aneurys... more We report the use of a custom made fenestrated endograft in two patients with aortic arch aneurysm and one patient with subacute type B aortic dissection (TBAD). A left common carotid-to-left subclavian artery bypass (LCCA-LSA) was performed in all cases. The innominate artery (IA) was cannulated from the right brachial access. The left brachial approach was used to catheterize the LSA and the LCCA artery through the LCCA-LSA bypass. A custom-made fenestrated endograft for the IA and LCCA was used in the first patient, a custom-made endograft with scallop for the IA and LCCA was deployed in the second case and a custommade fenestrated endograft for the IA and LCCA was used for the TBAD associated with two bare stents for the descending thoracic and abdominal aorta using the PETTICOAT technique. The LSA embolization was performed in all cases. The first procedure was complicated by the partial coverage of the LCCA ostium, corrected with the chimney technique.
