Purpose: We retrospectively evaluated the clinical, radiological and pathological features determ... more Purpose: We retrospectively evaluated the clinical, radiological and pathological features determining the prognosis of patients with nasopharyngeal carcinoma in Ankara Oncology Hospital, Turkey.
Material and methods: Two hundred and fifty-nine patients, 74 women and 185 males with nasopharyngeal carcinoma were treated between 1993 and 2008. All imaging data including CT and MRI were reevaluated according to the criteria which determine parapharyngeal, oropharyngeal, nasal, skull-base (bone)/sinus, infratemporal fossa, orbit, intracranial involvements and lymph node metastasis by our radiologists. The patients were restaged using the AJCC 2002 classification with these new radiological findings and clinical data base. We evaluated prognostic factors using univariate Kaplan- Meier and multivariate Cox regression analyses. Gender, age (40-year cut-off), histology, T- and N-stage, tumor size, regional involvement, radiotherapy and/or chemotherapy and response to therapy were studied as variables.
Results: Five-year disease-free and overall survival rates were 45 +/- 4 % and 72 +/- 3 % , respectively. We found that age, gender, WHO type, radiotherapy and/or chemotherapy, N-stage and response to therapy were significant prognostic factors on disease-free survival and overall survival. In the chemo-radiotherapy group, we did not detect any survival difference between patients given four or fewer chemotherapy courses.
Conclusions: Radiotherapy improved survival but chemotherapy, in the neoadjuvant and adjuvant setting, had no added effect to radiotherapy. N-stage and response to treatment were the most important independent predictors on survival. Age, gender, type, therapy and bone/sinus involvement were among the predictive factors on multivariate analysis, as well.

Also, it is to evaluate our series with previous series in this review. Material and Method: Review of previous series in which between January 1998 and July 2008, percutaneous biliary drainage was performed in 47 men and 29 women, 76 patients with malignant biliary obstruction. Ages were between 29 and 80 years, mean age: 55.0 years. Our patients had pain, emesis, vomiting, itching, weakness, and weight loss, while jaundice was the clinical symptom in almost all. Biliary stent was put in 5 (6.6%). Biloma was seen in 3 (4.0%). Success was evaluated by referencing direct bilirubin levels before and after drainage and clinical recovery. Bizim olgumuzda baş ağrısı ve görme bulanıklığı olması üzerine yapılan göz muayenesinde papil ödemle birlikte normal görme keskinliği saptanmıştır. Kranial ve orbita MRG sinde empty sella, optik sinirde tortiozite, skleral düzleşme, optik disk düzeylerinde protrüzyon, optik sinir etrafında artmış BOS izlenmiştir. MR venografisinde sağ sigmoid sinüste fokal bir alanda sinyal kaybı izlenmiş olup tromboza ait olabileceği düşünülmüştür. Olgumuzun MR bulguları eşliğinde intrakranial hipertansiyonun MR bulgularını sunmayı amaçladık.

Primary malignant tumors of the liver are rare in childhood and comprise 1-2% of all childhood ma... more Primary malignant tumors of the liver are rare in childhood and comprise 1-2% of all childhood malignancies. In this retrospective study the characteristics, treatment and outcome of childhood malignant hepatic tumors diagnosed and treated in a single center between 1996 and 2010 were evaluated. The pretreatment extent of disease (PRETEXT) system was used for staging and the SI-OPEL-3 or modified protocol for treatment. 20 children (13 male, 7 female), diagnosed and treated with primary malignant tumors of the liver (11 hepatocellular carcinoma [HCC], 9 hepatoblastoma [HBS]) were evaluated. In our series the cases with HCC were more than the cases with HBS. In cases with HCC, 8 had chronic hepatitis B infection. Serum alpha-fetoprotein levels were elevated in all cases with HBS and in six with HCC. According to the PRETEXT staging, 12 cases were stage (S) IV, 2 S III, 6 S II. Four had lung metastasis. Four cases with HBS, one patient with HCC are alive with no evidence of disease (19, 20, 21, 26, 102 months). The high incidence of hepatocellular carcinoma in our series is thought to be due to the high incidence of hepatitis B infection in Turkey. Children with chronic hepatitis B virus infection should be followed up keeping in mind the risk of HCC. Children with hemihypertrophia and other predisposing conditions should be followed up for the risk of liver tumors so that they can be diagnosed at early stages.

The purpose of this study was to analyze the possible relationships between computed tomography (CT) grading of SIJ arthritis and the effectiveness of intraarticular steroid injection treatment under CT guidance. PATIENTS AND METHODS A total of 61 patients with SIJ pain who were treated with CT guided intraarticular steroid injection were retrospectively reviewed. Visual analog scale (VAS) scores for pain control were recorded for short-term (day after injection, first week, third week) and long-term (sixth months and final control) follow-up times. In recent years, MRI has been gaining use as a modality that can provide important information about breast cancer lesions. Our aim was to determine if any significant relationship existed between dynamic MRI findings and factors which are currently being used to establish prognosis. Methods: Eighty-one patients with a diagnosis of invasive breast carcinoma and breast MRI were retrospectively reviewed and included in the study. Kinetic MRI features were determined on breast MR images, histopathological results were reviewed and prognostic factors were recorded. Results: Thirty-five patients' initial enhancement was lower than fifty percent, while 18 patients were between fifty and one hundred percent and 28 patients higher than one hundred percent. Our study is one of the latter, we could not identify any significant relationships between initial enhancement characteristics and dynamic curve types with any of the analyzed prognostic factors. Dıscussion and Conclusion: Although imaging can provide important data in the identification, diagnosis, and evaluation of many types of disease; imaging findings do not always correlate with patient clinic and prognosis. Up until now, studies on this matter have mostly found nothing of high significance; our study was no exception, however some of the data reported in this study can influence future studies.

Objective: Colon cancer, which is the most common type of cancer in the gastrointestinal system, ... more Objective: Colon cancer, which is the most common type of cancer in the gastrointestinal system, is a major cause of morbidity and mortality. It usually spreads over the liver. During the detection of the diseases, the existence of metastases in the liver cannot be overlooked. Additional liver metastases can also be observed in the monitoring period. Early diagnosis of liver metastases in colon cancer bears utmost importance. The use of perfusion changes in liver during the early diagnosis has been a popular research topic. Doppler Perfusion Index(DPI) can be defined as a term which occurs as a result of increasing hepatic arterial blood flow or decreasing portal venous blood flow and denotes the ratio of hepatic arterial blood flow to total liver blood flow. In this respect, the present study aims to detect the existence of micrometastasis using DPI in patients suffering from colon cancer and displaying no signs of visible liver metastasis. Material and Methods: We measured DPI index in 79 patients suffering from colon cancer and displaying no signs of visible liver metastasis and monitored them in terms of prospectively visible metastasis development. Results: During the monitoring process, 2 patients developed exitus and 9 patients developed metastasis. Thus, it can be stated that DPI values obtained from our patient group overlapped with the findings in the existing literature in terms of the early diagnosis of metastasis. Conclusion: Offering an inexpensive and non-invasive method for direct measurement, Duplex Color Doppler Ultrasonography(CDUS) method can be used in the early diagnosis of hepatic metastasis in patients with colorectal cancer.

In this study, we present the results of active H1N1 surveillance in 32 patients who were treated at our hematology/stem cell transplantation clinic between December 2009 and January 2010. We also report the clinical and laboratory features of patients with laboratory-proven disease and try to define the impact of novel H1N1 disease on their outcome. Eight patients in the hematology clinic and 7 patients in the hematology/stem cell transplantation unit tested positive for pandemic H1N1 infection. Patients were treated with oral oseltamivir for 5–15 days. In 10 patients the infection was limited to the upper respiratory tract. S... more A 5-year-old girl was admitted to our hospital due to fatigue and fever lasting for six months. She had systolic murmur in the mesocardiac and apex regions and hepatosplenomegaly. Laboratory evaluation revealed leukocyte and eosinophil counts of 176 and 144.32 x 109/L, 3.4% blasts in bone marrow and monosomy 8. She developed pulmonary, cardiac, nervous system, ocular and bone involvement. Upon diagnosis of "chronic eosinophilic leukemia, not otherwise specified" (WHO 2008 classification), she received methylprednisolone, vincristine, cytarabine and 6-thioguanine. After hematopoietic stem cell transplantation from a full-matched sibling was performed, the patient expired due to graft failure and septicemia.
