BACKGROUND: Acute myeloid leukemia (AML) cells harboring mutations in isocitrate dehydrogenase 1 ... more BACKGROUND: Acute myeloid leukemia (AML) cells harboring mutations in isocitrate dehydrogenase 1 (IDH1) and isocitrate dehydrogenase 2 (IDH2) produce the oncometabolite 2-hydroxyglutarate (2HG). This study prospectively evaluated the 2HG levels, IDH1/2 mutational status, and outcomes of patients receiving standard chemotherapy for newly diagnosed AML. METHODS: Serial samples of serum, urine, and bone marrow aspirates were collected from patients newly diagnosed with AML, and 2HG levels were measured with mass spectrometry. Patients with baseline serum 2HG levels greater than 1000 ng/mL or marrow pellet 2HG levels greater than 1000 ng/2 × 10 6 cells, which suggested the presence of an IDH1/2 mutation, underwent serial testing. IDH1/2 mutations and estimated variant allele frequencies were identified. AML characteristics were compared with the Wilcoxon test and Fisher's exact test. Disease-free survival and overall survival (OS) were evaluated with log-rank tests and Cox regression. RESULTS: Two hundred and two patients were treated for AML; 51 harbored IDH1/2 mutations. IDH1/2-mutated patients had significantly higher 2HG levels in serum, urine, bone marrow aspirates, and aspirate cell pellets than wild-type patients. A serum 2HG level greater than 534.5 ng/mL was 98.8% specific for the presence of an IDH1/2 mutation. Patients with IDH1/2-mutated AML treated with 7 + 3-based induction had a 2-year event-free survival (EFS) rate of 44% and a 2-year OS rate of 57%. There was no difference in complete remission rates, EFS, or OS between IDH1/2-mutated and wild-type patients. Decreased serum 2HG levels on day 14 as a proportion of the baseline were significantly associated with improvements in EFS (P = .047) and OS (P = .019) in a multivariate analysis. CONCLUSIONS: Among patients with IDH1/2-mutated AML, 2HG levels are highly specific for the mutational status at diagnosis, and they have prognostic relevance in patients receiving standard chemotherapy.

Renal involvement in Hansen's disease was evaluated in 94 Portuguese patients, average ag... more Renal involvement in Hansen's disease was evaluated in 94 Portuguese patients, average age and duration of disease of 47.6 and 6.8 years respectively. Sixty-seven were studied retrospectively and 27 prospectively; renal biopsy was obtained in 4, fat-tissue needle aspiration for amyloidosis in 20, and tubular function was tested in ten. Mild proteinuria and/or haematuria was found in 33 patients, the severity increasing during erythema nodosum leprosum reactions, but without overt nephritic or nephrotic syndrome. Two patients had renal amyloidosis on biopsy and two more were confirmed by fat biopsy, a 10.5% incidence in those studied prospectively; all but one were of the lepromatous type, with frequent bouts of erythema nodosum leprosum. The two other renal biopsies showed mesangial glomerulonephritis, and one unexplained acute tubular necrosis; none had immune deposits by immunofluorescence. Proximal acidification was always normal, distal acidification tested by bicarbonate infusion was abnormal in one of nine patients, and six of nine patients had concentration defects. Leprosy causes frequent urinary sediment changes and concentration defects, usually without clinical expression; proteinuria and/or glomerular involvement is mainly due to amyloidosis.

Protein equalization with dithiothreitol, protein depletion with acetonitrile and the entire prot... more Protein equalization with dithiothreitol, protein depletion with acetonitrile and the entire proteome were assessed in conjunction with matrix assisted laser desorption ionization time of flight mass spectrometry-based profiling for a fast and effective classification of patients with renal insufficiency. Two case groups were recruited as proof of concept, patients with chronic glomerulonephritis and diabetic nephropathy. Two key tools were used to develop this approach: protein concentration with centrifugal concentrator tubes with 10KDa cut-off membranes and chemical assisted protein equalization with dithiothreitol or chemical assisted protein depletion with acetonitrile. In-house developed software was used to apply principal component analysis and hierarchical clustering to the profiles obtained. The results suggest that chemical assisted protein equalization with dithiothreitol is a methodology more robust than the other two ones, as the patients were well grouped by principal component analysis or by hierarchical clustering.

Problematiza a polissemia da educação em direitos humanos e a pretensa universalidade que marca direitos como humanos; a partir daí, argumenta que essa se dá como articulação da diferença e como enunciação cultural. Com balizamento nesse sentido, defende que a configuração pedagógica dos direitos humanos se dê a partir dessa negociação/articulação. Palavras-chave-educação em direitos humanos; negociação da diferença; hegemonia ABSTRACT-The article aims to analyze the different meanings that underlie the discussion of the appropriation of human rights issues in education, questioning the policy-making curriculum for Human Rights Education. From there on we argue that this occurs as the difference in articulation and enunciation as cultural and buoyed by this sense, we argue that the educational setting of human rights is to be given from this negotiation and or articulation.

Carbon dioxide diffusion into n-decane inside cylindrical and square glass capillary tubes has be... more Carbon dioxide diffusion into n-decane inside cylindrical and square glass capillary tubes has been modeled [1,2], with two different models for each tube and the convective model for the square tube depended on the results of the cylindrical one. For those models, the liquid phase density was always considered constant and its value was adjusted from the experimental data of gas-liquid interface position. This approach was done using the diffusivities obtained by correlations which modify the infinite dilution diffusion coefficient using a thermodynamical factor. Now, the liquid phase density is considered variable on time with perfect mixing inside the phase and an effective diffusivity can be determined. This effective diffusivity involves the molecular and convective contributions to the global mass transfer. Both interface displacements (inside cilyndrical and square tubes) can be modeled using the same model without dependency between their results. The terms inside the finite difference matrix for the liquid phase are not constant, because they depend on the solute concentration and on the liquid density then an iterative calculation for the matrix coefficients must be done in each timestep. A partially implicit model considers this iterative calculation keeping the liquid density value for the previous time (j). A fully implicit model considers this iterative calculation keeping the liquid density value for the present time (j+1). It was showed that the model results, adjusted to the experimental interface position values, predicted effective diffusivities which are variable on time. The simulation time (76 min) for the fully implict numerical model is higher than the simulation time (62 min) for the partially implicit numerical model. It was found that the type of numerical solution scheme affects the results (up to 5% deviation) for the square capillary model but it doesn´t change the cilindrical capillary model results.

In advanced cirrhosis, increased levels of vasodilators and impaired cardiac compensatory respons... more In advanced cirrhosis, increased levels of vasodilators and impaired cardiac compensatory response decrease effective arterial blood volume, causing vasoconstriction of renal arteries and kidney failure in up to 40% of patients after 5 years of follow-up. Hepatorenal syndrome (HRS) diagnostic criteria are: cirrhosis with ascites; serum creatinine (SCr) > 1.5 mg/dL (with no improvement 2 days after diuretic withdrawal and albumin administration). Shock, nephrotoxics and acute parenchymal kidney disease must be excluded. The HRS is classified in: type 1, defined by a 100% increase in SCr to > 2.5 mg/dL in < 2 weeks, and type 2, with a slower and milder decrease in kidney function. Type 3 HRS is an emerging concept, referring to HRS in patients with coexistent kidney disease. Left untreated, average survival of type 1 HRS is 2 weeks whereas in type 2 it is 6 months. Treatment of HRS lies on reversal of the hepatic disease or liver transplantation (combined liver-kidney transplant may be appropriate for patients who have been on renal replacement therapy (RRT) for more than 8 weeks). However, with today's available therapy, there may be reversibility of HRS without liver transplant. Type 1 HRS is treated with vasoconstrictors (mainly terlipressin; noradrenalin may be an alternative in patients in intensive care units) and albumin. Reversal of HRS occurs in about half of patients. If SCr does not decrease and patients have classic indications for dialysis, RRT can be used as a second-line treatment until liver recovery or transplant. MARS (molecular readsorbent recirculating systems) and Prometheus systems should be considered experimental. Type 2 HRS treatment is based on repeated large-volume paracentesis and albumin administration. If ineffective, vasconstrictors are used. Since renal impairment is mild, RRT is not indicated. If liver recovery/transplant are unfeasible, patient's treatment should avoid futilities.

Dialysis patients appear to be predisposed to infection due to alterations in primary host defence, advanced age and the presence of comorbid conditions, malnutrition and invasive dialysis procedures. Bacteraemia is seldom a manifestation of infection in peritoneal dialysis patients and is usually secondary to catheter-associated peritonitis. Campylobacter fetus, unlike more common isolates of the same genus, is usually grown from blood samples, without accompanying enteritis, in patients with serious underlying conditions. The authors present a case of Campylobacter fetus bacteraemia in a peritoneal dialysis patient.

As the number of diagnostic and interventional angiographies and computerized tomography increases in clinical practice and higher doses of contrast media are administered to sicker and older patients, contrast-induced acute kidney injury is an increasing problem. Focaliza textos voltados à formação continuada de educadores e à formulação de diretrizes curriculares de Educação em Direitos Humanos, produzidos em função de projeto implementado com apoio do Ministério da Educação. Argumenta favoravelmente à assunção de uma noção de Direitos Humanos como espaço de expressão da diferença, arena de dissenso, a partir do que se questionam as práticas prescritivas afirmativas de modelos universalizados que caracterizam políticas curriculares, perspectiva que pode ser identificada na experiência analisada de constituição do currículo de Educação em Direitos Humanos.

Background and Objectives: As a result of improved clinical and quality-of-life outcomes, in-cent... more Background and Objectives: As a result of improved clinical and quality-of-life outcomes, in-centre nocturnal haemodialysis emerges as one of the alternatives to conventional haemodialysis. Even today, little is known about the flow of patients through in-centre nocturnal haemodialysis programmes or about patient's survival or technique failure. This study sought to address this gap in knowledge. Materials and Methods: Retrospective, descriptive study that included all patients in in-centre nocturnal haemodialysis programme, between January 1995 and December 2011, which involved 51 patients. Descriptive data at baseline were presented as means or percentages of the total. We used Kaplan-Meier survival curves for patient survival. Results: Patients underwent for more than 5.5 hours thrice weekly sessions and remained in the technique for a mean period of 59.5 ± 73 months. The annual input and output of patients in the programme was on average of to 2.1 and 2.5 patients, respectively. Forty-three patients discontinued the technique: kidney transplantation in 24, conventional haemodialysis in 11 and peritoneal dialysis in three patients, with mean times in in-centre nocturnal haemodialysis of 37.4 ± 46.4, 85.3 ± 93 and 59.7 ± 79.7 months, respectively. Throughout the period under review, five patients died (9.8%). Unadjusted survival at 20 years was 65% in patients who remained in in-centre nocturnal haemodialysis. Conclusion: In-centre nocturnal haemodialysis is associated with a higher survival rate, notwithstanding the possibility of selection bias resulting from patient's profile that opts for this modality. The relative contribution of patient selection versus effect of therapy on outcomes requires evaluation in future prospective clinical trials.

Histoplasmosis is a clinically relevant fungal infection in immunosuppressed patients, in whom it... more Histoplasmosis is a clinically relevant fungal infection in immunosuppressed patients, in whom it may present as a serious disease. We describe the case of a renal transplant recipient who presented with malaise, dry cough, severe weight loss, abdominal pain and watery diarrhoea. The laboratory workup revealed anaemia and elevated C reactive protein. Chest X-ray showed bilateral reticulonodular infiltrates, and computed tomography scan of the chest and abdomen revealed diffuse thickening of the inter-and intralobular septs, suggestive of lymphangitic infiltration. The abdomen had multiple lymphadenopathies along the mesenteric vessels. Endoscopic evaluation disclosed disseminated ulcers throughout the entire gastrointestinal tract, and biopsies confirmed the diagnosis of histoplasmosis. Despite adequate treatment with liposomal amphotericin B, the patient had a fatal outcome. We describe this case because of the rarity of gastrointestinal presentation in the context of disseminated histoplasmosis, and we also speculate about the transmission through the allograft. It highlights the importance of considering this once geographically limited disease in challenging cases, even in non-endemic areas.

Depletion of high-abundance proteins is regarded as a critical sample preparation step for proteomic analyses and profiling strategies [1] The present work intends to be a step further towards a fast, reliable and cheap method to classify patients in peritoneal dialysis as well as to as to help in medical diagnosis.

Several forms of glomerulopathies have been described in patients who use intravenous heroin, inc... more Several forms of glomerulopathies have been described in patients who use intravenous heroin, including focal glomerulosclerosis, membranous nephropathy associated with chronic hepatitis B antigenemia, and immune complex proliferative glomerulonephritis. This report describes a patient who was a heroin addict for 15 years and who developed membranoproliferative glomerulonephritis and cutaneous vasculitis due to mixed cryoglobulinemia; he also had a positive hepatitis C virus (HCV) antibody. Recently, an association has been described between &quot;essential&quot; mixed cryoglobulinemia and chronic HCV infection, which has a high prevalence in intravenous drug addict population. It is possible that mixed cryoglobulinemia-associated glomerulonephritis, occurring with chronic HCV infection, came to be considered a major cause of renal disease in heroin abusers. The possibility that HCV infection can be responsible for other types of renal involvement in intravenous drug addicts deserves further attention.

Abordando el conflicto como una producción del encuentro de la diferencia, Mouffe lo comprende como un aspecto indeleble en la constitución del social. En este encuentro con la autora, buscamos reflexionar algunos temas y problemáticas centrales de su trabajo, y las implicaciones de su teoría en el campo educacional contemporáneo.

These students participated actively in laboratory experiments, practical workshops and computer classroom sections. They expanded their knowledge and written practice using the resources compiled in a web page and sending some of their compositions via e-mail. From the paragraphs created and corrected by students themselves, it is possible to assert that the "Step Up to Writing" program and content-based instruction provided them with a very useful path to improve both their writing skills and their knowledge about food.

The rate of infectious morbidity and mortality is much higher when catheters are used than when patients are dialyzed through grafts or native fistulas, and it is generally agreed that implementing appropriate preventive measures would do more to lower its incidence. Methods: A prospective, randomized, open-label, long-term follow-up clinical trial was conducted to compare the efficacy of dressing the exit site with antibiotic ointment (AO) versus catheter antimicrobial locking (AL) in preventing catheter-related bacteremia (CRB), both associated with strict standard precautions and appropriate catheter care.
