The aim of this study was to evaluate the treatment needs of first permanent molars in a group of schoolchildren in Mashhad. This cross-sectional descriptive study was carried out on 700, 7-9 year-old students in primary schools in Mashhad. The schools were randomly selected from each district. Treatment needs and DMFT of first perma-nent molars were calculated. Data was analyzed using ANOVA, Chi-Square and t-test. A total of 95.3% of the children required dental treatment. Fissure sealant application and filling were the treat-ments most required in all age groups. The mean DMFT of first permanent molars was 1.31±1.4. It was significantly higher in girls than boys (P=0.040). Great treatment needs and caries prevalence in permanent first molars in Mashhad schoolchildren show that dental caries is still a serious problem in the children of our society; theref...

Over the past decade, paediatric dentistry has primarily shifted its focus towards the preservation of vital tissues and improvement of treatment outcomes, and has benefitted immensely from the introduction of effective and safe bioactive endodontic materials. Since 1993, the year in which mineral trioxide aggregate was introduced, hydraulic calcium silicate–based biomaterials have made a significant impact on endodontic treatments. There is a growing body of evidence demonstrating that hydraulic calcium silicate cements are beneficial for both the least aggressive and most invasive type of vital pulp therapies, as well as for the management of non-vital primary/permanent young teeth. Moreover, clinicians’ skills and knowledge regarding the applications of hydraulic calcium silicate–based biomaterials play a crucia...

In this study, PubMed, ISI (all databases), Scopus, and Cochrane Database were searched according to the selected keywords up to May 11, 2016. The full texts of published articles that appeared to meet the primary criteria for inclusion in this study were obtained. Due to the variation in the methods used, the studies were divided into 2 groups: Group 1-studies that evaluated the micro-tensile bond strengths (MTBSs) of two-step etch-and-rinse adhesives and two-step self-etch adhesives and Group 2-studies that evaluated the MTBSs of two-step etch-and-rinse adhesives and one-step selfetch adhesives. The initial search yielded 1447 publications. After a methodological assessment, 8 publications were selected. The results of this study showed that the MTBS of the etch-and-rinse adhesives and the two-step self-etch adhesives were similar (P = 0.34), and both were significantly higher than that of the one-step self-etch adhesive (P = 0.001). This meta-analysis showed that the application of two-step self-etch adhesives performs well when used for primary dentin and can be used to save time in pediatric dentistry.

External inflammatory root resorption (EIRR) is one of the common complications following dental ... more External inflammatory root resorption (EIRR) is one of the common complications following dental trauma which when remained untreated, may lead to tooth loss. Successful treatment outcomes depend on elimination of bacteria from root canal system and apical sealing. This case presents the endodontic management of an EIRR that was nonresponsive to calcium hydroxide (CH) therapy. An 11-year-old boy was referred for management of a traumatized maxillary central incisor. Tooth #8 was symptom-free, nonresponsive to vitality pulp tests and had an immature root with sever EIRR. Using chemomechanical debridement and CH dressing, the treatment was initiated. The tooth was remained asymptomatic; however, after five weeks the size of periradicular lesion increased and intracanal exudate was present, signifying a resistant endodontic infection. In second appointment, double antibiotic paste (DAP; ciprofloxacin/metronidazole) was applied to the canal. Eight weeks later, the tooth continued to be ...

Methods: Five electronic databases (PubMed, ISI web of science, Scopus, Cochrane and OVID) by keywords based on the PICO were searched to identify all the clinical trials with topical and systemic therapeutic interventions aimed at treating or preventing recurrence published in Persian or English that related to the effects of drugs of natural herbal origin for the treatment of recurrent aphthous stomatitis from 2010 to 2015. A structured and standardized form has been used to extract data. Title, journal, year of publication and the field of the first author and corresponding author were recorded. Standard tools of risk of bias used in the systematic review studies that has been developed by the Cochrane (Last update 2011) was applied to investigate bias in the studies. Results: In the current study a total of 33 trials were analyzed. These studies were assessed the effectiveness of 29 different herbal drugs for RAS treatment. Only five studies were assessed as being at medium risk of bias. No study had a low risk of bias. There were insufficient evidences to support or refute the use of these interventions. Conclusion: In summary, current data suggested that there was a beneficial effect in using topical treatments with plant origin for RAS and no study did not report any side effects. Though due to very weak report and disparate studies, acknowledging the fact that which herbal treatment was the best and most effective treatment was not possible.

The aim of the present study was to evaluate the accuracy of root canal length measurement with Root ZX II apex locator and rotary system in pulpectomy of primary teeth. Material and Methods: In this randomized control clinical trial complete pulpectomy was performed on 80 mandibular primary molars in 80, 4-6-year-old children. The study population was randomly divided into case and control groups. In control group conventional pulpectomy was performed and in the case group working length was determined by electronic apex locator Root ZXII and instrumented with Mtwo rotary files. Statistical evaluation was performed using Mann-Whitney and Chi-Square tests (P < 0.05). Results: There were no significant differences between electronic apex locator Root ZXII and conventional method in accuracy of root canal length determination. However significantly less time was needed for instrumenting with rotary files (P=0.000). Conclusions: Considering the comparable results in accuracy of root canal length determination and the considerably shorter instrumentation time in Root ZXII apex locator and rotary system, it may be suggested for pulpectomy in primary molar teeth.

The aim of this study was evaluation of the effect of CHX on shear bond strength (SBS) of self-etch (SE) and etchand-rinse (E&R) bonding systems to the primary teeth dentin. Materials and methods: In this in vitro study, 40 freshly extracted primary molars collected and were cut mesiodistally, and then the buccal and lingual surface enamel was grounded. The specimens were randomly divided into two groups: I: Single Bond adhesive and II: Clearfil SE Bond adhesive. Then, each group was divided into two subgroups: On the buccal surface of each sample (I 1 , II 1), CHX was applied for 30 seconds and on the lingual surface of (I 2 , II 2 groups), CHX was not applied. The SBS (MPa) of samples was assessed using the Zwick universal testing machine. Data were analyzed by paired t-test and independent t-test using Statistical Package for the Social Sciences (SPSS), version 16. Results: The mean SBS was significantly higher in the Single Bond group with CHX (66.45 ± 8.3) than without CHX (25.43 ± 12.94) in the nonaging group (p = 0.002). There was no significant difference in the mean SBS in the Clearfil SE Bond group with CHX and without CHX in the aging and nonaging groups. Conclusion: CHX had positive influence on immediate SBS of Single Bond adhesive to primary teeth dentin. In addition, no positive effect has been seen in the Clearfil SE Bond system.

The aim of this systematic review and meta-analysis was to evaluate the ability of glass ionomer cements (GICs) and resin-based sealants (RBSs) to prevent the occurrence of caries and their retention in standards-based clinical studies. Types of Studies Reviewed. The authors conducted a literature search (from database inception through September 20, 2017) to identify studies for inclusion in this systematic review. The authors assessed the quality of the evidence with the modified Jadad scale and performed the meta-analysis by using a random-effects model. Results. The authors considered 20 studies on caries prevention and 28 studies on retention that met the inclusion criteria for the meta-analysis. The results of the meta-analysis for caries development showed no significant difference (odds ratio, 0.938; 95% confidence interval, 0.647 to 1.359; P ¼ .734). However, the result for the retention rate showed the advantage of RBSs (odds ratio, 6.006; 95% confidence interval, 3.226 to 11.183; P ¼ .000). Conclusions and Practical Implications. There was no difference between the percentage of caries development with use of GICs as fissure sealing material compared with that for the conventional RBSs, but the retention rate of conventional RBSs was much higher than that of the GICs.

OM affects more than 75% of patients undergoing chemotherapy and represents a significant burden to patients and caregivers. We performed a systematic review and meta-analysis of published studies to investigate the effects of agents and techniques in reducing OM. Procedure: This systematic review investigated, critically appraised, and rated the evidence on agents used to manage OM in children undergoing cancer therapy. A comprehensive search of the relevant literature was performed from January 2006 to December 2017. MEDLINE, Scopus, the Cochrane Database of Systematic Reviews, EMBASE, and the Web of Science were searched. Nine eligible studies were identified. Using random-effects models, standardized mean difference was estimated between treated and control groups across all studies. The Cochran test and the I 2 index were performed for heterogeneity between studies. The significance level was set at P = 0.05. Results: Palifermin reduced the incidence (OR = 4.131, P = 0.000), duration (St diff mean = 0.803, P = 0.000), and severity (St diff mean = 0.637, P = 0.000) of OM in pediatric cancer patients significantly. However, the laser did not show significant efficacy in decreasing the incidence rate of OM (OR = 2.870, P = 0.364). Conclusion: This review provided a comprehensive examination of available options for children who have OM. The results support the possibility of a positive effect of palifermin on reducing OM in children receiving cancer therapy.
