La diferenciación entre abscesos y metástasis cerebrales en pacientes con algún grado de inmunosu... more La diferenciación entre abscesos y metástasis cerebrales en pacientes con algún grado de inmunosupresión y con neoplasias hematológicas resulta en ocasiones difícil. Los estudios funcionales como la tomografía de emisión de fotón único (SPECT) y la tomografía por emisión de positrones (PET) son herramientas diagnósticas eficaces para este propósito. Presentamos el caso de una paciente con diagnóstico de leucemia mieloide crónica en crisis blástica a quien se le informó múltiples abscesos cerebrales; diagnóstico basado en que no existió captación de 99m Tc -MIBI en SPECT cerebral y en la buena respuesta al tratamiento antimicrobiano en ocasión en que los estudios de tomografía computarizada (TC) cerebral y resonancia magnética (RM) cerebral no fueron concluyentes. Teniendo en cuenta los resultados de la SPECT se decidió postergar el esquema de quimioterapia, ya que esta podría desencadenar complicaciones probablemente fatales en la paciente.

In order to evaluate the capacity of myocardial perfusion scintigraphy with Tc99m-MIBI to predict... more In order to evaluate the capacity of myocardial perfusion scintigraphy with Tc99m-MIBI to predict the evolution of women referred for evaluation of precordial pain, the rates of cardiovascular events at one year were studied in a group of patients with normal scintigraphy (group 1: 17 patients) and in another with reversible perfusion defects (group 2: 17 patients). The one-day protocol was applied for scintigraphy with Tc99m-MIBI (rest/stress) with combined stress (ergometric bicycle plus 0.28 mg/Ig of dipyridamole) in those cases where it was necessary. A quantitative and qualitative analysis of the regional uptake was made. Patients from group 1 were younger than those from group 2 (average age: 49 vs 55, respectively, p = 0.04) and they had less hypercholesterolemia (112% vs 59%, respectively, p = 0.01). To assess the temporal parameters of ventricular synchronization in the normal heart by a third harmonic (3H) Fourier phase analysis in a RNV and introduce this technique in our center. Thirty normal subjects (19 men and 11 women) were included. An equilibrium RNV was performed in 35 degree left anterior oblique projection with 10 degree caudal tilt. The onset (T0); mean time (T(m)); total contraction time (T(t)); final time (T(f)) and propagation time (T(p)) for right (RV) and left ventricle (LV); as well as total propagation time (T(TP)); interventricular time (T(RV-LV)) and septum-lateral wall conduction time (T(S-LW)) were measured on the 3H Fourier histogram of the time-activity curve. A discriminant method for optimizing activity in nuclear medicine studies is validated by compari... more A discriminant method for optimizing activity in nuclear medicine studies is validated by comparison with ROC (received operating characteristic)-curves. The method is tested in 21 single photon emission computerized tomography (SPECT), performed with a cardiac phantom. Three different lesions (L 1 , L 2 and L 3) were placed in the myocardium-wall by pairs for each SPECT. Three activities (84, 37 or 18.5 MBq) of 99mTc were used as background. Linear discriminant analysis was used to select the parameters that characterize image quality among the measured variables in the images [(Background-to-Lesion (B/L i) and Signal-to-Noise (S i /N) ratios)]. Two clusters with different image quality (P=0.021) were obtained. The ratios B/L 1 , B/L 2 and B/L 3 are the parameters used to construct the function with 100% of cases correctly classified into the clusters. The value of 37 MBq was the lowest tested activity for which good results for the B/L i ratios were obtained. The result coincides with the applied ROC-analysis (r=0.89).
CK2 represents an oncology target scientifically validated. However, clinical research with inhib... more CK2 represents an oncology target scientifically validated. However, clinical research with inhibitors of the CK2-mediated phosphorylation event is still insufficient to recognize it as a clinically validated target. CIGB-300, an investigational peptide-based drug that targets the phosphoaceptor site, binds to a CK2 substrate array in vitro but mainly to B23/nucleophosmin in vivo. The CIGB-300 proapoptotic effect is preceded by its nucleolar localization, inhibition of the CK2-mediated phosphorylation on B23/ nucleophosmin and nucleolar disassembly. Importantly, CIGB-300 shifted a protein array linked to apoptosis, ribosome biogenesis, cell proliferation, glycolisis, and cell motility in proteomic studies which helped to understand its mechanism of action. In the clinical ground, CIGB-300 has proved to be safe and well tolerated in a First-inHuman trial in women with cervical malignancies who also experienced signs of clinical benefit. In a second Phase 1 clinical trial in women with cervical cancer stage IB2/II, the MTD and DLT have been also identified in the clinical setting. Interestingly, in cervical tumors the B23/nucleophosmin protein levels were significantly reduced after CIGB-300 treatment at the nucleus compartment. In addition, expanded use of CIGB-300 in case studies has evidenced antitumor activity when administered as compassional option. Collectively, our data outline important clues on translational and clinical research from this novel peptide-based drug reinforcing its perspectives to treat cancer and paving the way to validate CK2 as a promising target in oncology.

ABSTRACT Con el propósito de explorar si el diagnóstico de viabilidad puede lograrse con un radio... more ABSTRACT Con el propósito de explorar si el diagnóstico de viabilidad puede lograrse con un radiofármaco marcado con tecnecio, se les realizó a 25 pacientes con infarto del miocardio (IM) previo, una gammagrafía de perfusión miocárdica con Tc99m -Tetrofosmín en protocolo de esfuerzo/reposo, y además en una tercera parte se les inyectó el fármaco radiactivo 55 min después de administrar 5 mg de nisoldipina oral, una dihidropiridina vasodilatadora coronaria. El resultado del examen tetrofosmín-nisoldipina se comparó con una gammagrafía de perfusión miocárdica con talio realizada en los mismos pacientes entre 20 y 35 días después de concluida la primera. El diagnóstico de viabilidad se basó en un aumento de la captación de radiofármaco dentro del área del infarto post-nisoldipina en el caso del tetrofosmín y tras la reinyección en el caso del talio. De los 25 pacientes incluidos en el estudio, 18 presentaron imágenes de viabilidad, 17 de ellos con tetrofosmín (post-nisoldipina) y los 18 con reinyección de talio, la correlación obtenida entre ambos métodos fue del 96 %, demostrando que la nisoldipina es una opción para determinar la existencia de viabilidad. With the objective of finding out if viability diagnosis based on a techtenium-labelled radiopharmaceutical may be made, myocardial perfusion imaging Tc99-Tetrofosmin in work/ rest protocole was performed on 25 patients with previous myocardial infarction and also, one third of them was injected the radiopharmaceutical 55 minutes after oral administration of 5mg of nisoldipine, which is a coronary vasodilator dihydropiridine. The outcome of tetrofosmin-niseldipine analysis was compared with myocardial perfusion imaging with Thalium performed on the same patients 20-35 days after the first test. The viability diagnosis was based on the increase of radiopharmaceutical uptake within the infarction area after nisoldipine administration in the case of tetrofosmin, and after Thalium reinjection. Out of 25 patients in the study, 18 showed viability images with Thalium reinjection and 17 with tetrofosmin (after nisoldipine). The correlation between both methods was 96 % proving that nisoldipine is an alternative for determining myocardial viability.
Monitoring the quality of instrumentation used in nuclear medicine is mandatory to guarantee the ... more Monitoring the quality of instrumentation used in nuclear medicine is mandatory to guarantee the clinical efficacy of medical practice. A national program for the quality control of nuclear medicine instruments was established in Cuba and was certified and approved by the regulatory authorities. The program, which establishes official regulations and audit services, sets up educational activities, distributes technical documentation, and maintains a national phantom bank, constitutes a valuable and useful tool to guarantee the quality of nuclear medicine instrumentation.
