En su opinion es una intervencion que ofrece buenos resultados en dichos estadios para trabajadores manuales de fuerza. EnglishThe authors made a review about 16 patients with Kienbock disease, in late stage (III-IV of de Decoulx clasification), operated by Graner's procedure, this results was excellent about pain, good strength and poor movility. They beleave is a good procedure in that stage for strengh manual workers.

Our experience on the management of 6 cases with bilateral chronic exertional compartment syndrom... more Our experience on the management of 6 cases with bilateral chronic exertional compartment syndrome of the legs is presented. The diagnosis was confirmed by the measurement of the compartment pressure with slit catheter. Intramuscular pressure was considered abnormal when the basal pressure was not regained 15 minutes after the exercise with consistent symtomatology. Double complete subcutaneous fasciotomy was performed in the 6 patients obtaining complete recovery of the symtoms and improvement of the performance in all of them. Patients started training six weeks after surgery. Neither complication or relapse was found during a mean follow-up of 2 years (1-3).

The authors made a long-term versus short-term review of results. They have found a worsening of results in the total longterm review. They observe a worsening of the stability (90.47% excellent results in short-term but 58.62% in long-term) (9.52% bad results in short-term but 31.03% in long-term). There are an increase of range mobility (57.14% excellent in short-term versus 82.75% in long-term). The function remain unchanged.

Para el E.M.G. se ha conseguido una exactitud global del 83% y para la saculorradiculografia del 79%. La saculorradiculografia ha demostrado mayor exactitud para la raiz S1 que paral a L5 (85,7% frente a 70,4%) y el E.M.G. mayor para la L5 que para la S1 (88,6% frente a 78,5%). El E.M.G. tiene una especial utilidad para el diagnostico y despistaje de neuropatias y para la comprobacion de recuperacion post-quirurgica de lesiones sin axonotmesis. La exactitud de los resultados en el E.M.G. estan en relacion directa con la habilidad del examinador a la hora de conseguir e interpretar los resultados. The patient's course is described and available publications are fully reviewed. The authors believe that in spite of the high rate of necrosis observed in talus enucleations, it is valid to attempt treatment designed to prevent necrosis, reserving arthrodesis for those cases where necrosis has already occurred or where walking is impaired.

Posterior sternoclavicular dislocation is a relatively uncommon lesion, but must always be consid... more Posterior sternoclavicular dislocation is a relatively uncommon lesion, but must always be considered in the event of trauma to the scapulothoracic area in which initial radiology does not show signs of fracture. Its diagnosis and treatment must be carried out promptly because of the possible serious complications that may occur through the clavicle compressing nearby structures. The authors report two cases, which were diagnosed by CT-scan. In the first case, treatment consisted in orthopedic reduction, while in the second case open reduction and fixation with Kirschner wires was required. The result was satisfactory in both cases, and the patients remain asymptomatic three and five years after trauma.

We report an eight year-old female with trisomy 21 referred to our clinic for limitation of wrist... more We report an eight year-old female with trisomy 21 referred to our clinic for limitation of wrist mobility. The patient had been diagnosed and treated by polyarticular juvenile chronic arthritis for six months. Clinical and radiological study revealed volar radiocarpal dislocation of the left wrist. She was treated surgically by open reduction, temporary K-w fixation with six weeks of immobilization. The dislocation relapsed but the joint remained painless. One year later she had 5º of dorsal flexion and 25º of volar flexion and absence of pain. In spite of this situation the family said that the girl lived normal life without any limitation because of her condition, refusing further treatment. Eight years later the girl remains asymptomatic and does a normal life without limitation because of her condition. The patient and her family refused any treatment. Based in the failure of surgical treatment performed in our case we think that abstention could be the most reasonable option, mainly in the immature patients.

Resumen.-El objetivo del presente trabajo es valorar clínica y radiológicamente la evolución de 1... more Resumen.-El objetivo del presente trabajo es valorar clínica y radiológicamente la evolución de 125 cotilos de Mittelmeier implantados en nuestro servicio, con un seguimiento medio de 8 años (5,5-14). En toda la serie se ha utilizado un mismo tipo de cotilos, aunque se han utilizado 2 vástagos, los de segunda y los de tercera generación. El comportamiento ha sido satisfactorio en el 91% de los de tercera generación y el 57% en los de segunda. Se comprobó una inclinación y recubrimientos progresivos y una emigración horizontal y vertical significativas de los cotilos. No se encontró relación entre los resultados clínicos y las alteraciones posicionales. La movilización del cotilo aparece en nuestra serie mucho más frecuentemente que en la mayor parte de la literatura. Las emigraciones horizontal y vertical pueden ser debidas a los diferentes coeficientes de elasticidad de la cerámica y el hueso sin implicar necesariamente malos resultados. Como conclusión, el comportamiento de los cotilos de Mittelmeier ha sido en general satisfactorio a pesar de las emigraciones, que han tenido escasa repercusión clínica. MEDIUM TO LONG-TERM RESULTS OF MITTELMEIER CUPS Summary.-This study was aimed at evaluating clinically and radiologically the results of 125 Mittelmeier cups implanted in our institution, with a mean follow-up of 8 years (5.5-14). The same type of cup was used in the whole series, combined with two types of femoral stem from the 2 nd and 3 rd generations. The results were satisfactory in 91 % of cases treated with the 3 rd generation and 57% in those cases treated with the 2 nd generation of implants. Progressive inclination and coverage were detected with both horizontal and vertical migration of the cups. No relationship was found between clinical results and the displacements observed. In this series, migrations of the cup were more frequent than those published by other authors. Horizontal and vertical migratrion may be due to the difference between the coefficients of elasticity of ceramic and bone. Cup migration did not imply poor results. In conclusion, the overall results obtained with the Mittelmeier cups were satisfactory, in spite of migration. The clinical repercussion of cup migrations was minimal. In contrast to acute compartment syndrome, CECS is generally not related to trauma, and is often suspected in the outpatient setting by primary care physicians, podiatrists, sports medicine clinicians, and orthopedic surgeons. The diagnosis of CECS is often overlooked because patients avoid or withdraw from exacerbating physical activities instead of seeking treatment for their symptoms from a health care professional. A thorough history and physical examination of an individual with activity-related lower leg pain is necessary for correct diagnosis to occur. Appropriate diagnostic testing with measurement of intracompartmental pressures reliably confirms the diagnosis of CECS. Nonoperative treatments of CECS rarely leads to complete resolution of symptoms or an individual's ability to return to previous levels of recreational or athletic act...

Only 2 cases of monostotic sacral involvement were published previously. The lesion was detected in a 42-year-old man suffering from lumbosacral pain after minimal trauma. Radiographic studies revealed cystic images on both alae of the sacrum, with internal condensations and some liquid contents. A trephine biopsy did not produce enough diagnostic data, and a posterior surgical approach was elected. Curettage and refilling with allograft were performed. The pathologic anatomy study diagnosed a fibrous dysplasia. Two years after surgery, the patient was asymptomatic.

Intradural Disc Herniation and Epidural Gas: Something More Than a Casual Association?

The case of a patient with an intradural disc herniation associated with gas in the spinal canal ... more The case of a patient with an intradural disc herniation associated with gas in the spinal canal is presented. To alert spine surgeons to this potential association. This association was described previously in five patients, a fact that seems surprising given the relative rarity of both intraspinal gas and intradural herniations. The case is presented of a female patient with lumbosciatic pain who developed an incomplete cauda equina syndrome. An asymmetric discopathy of the L2-L3 space and a gas bubble with disc material within the spinal canal was noticed in the radiologic explorations. The literature and the authors' experience are reviewed with the aim of confirming the frequency of intradural herniation in association with gas in the spinal canal. A laminoarthrectomy of the involved space was performed followed by direct intradural examination, which revealed a disc fragment that was excised. An instrumented L2-L3 arthrodesis was performed. Postoperative evolution was satisfactory. To date, the authors have found this association in 2% of the patients with intraspinal gas. The potential presence of an intradural disc herniation must always be considered when performing an open discectomy on a patient whose CT scan study shows the presence of epidural gas. This association is particularly striking given the relative rarity of intradural herniations and intraspinal gas. In the event that no clear disc herniation was found, an intradural examination may be indicated to justify clinical signs and symptoms or previous radiologic studies.
