Evaluation of the Left Ventricular Diastolic Function Using Three-Dimensional Echocardiography

The aim of this study was to evaluate the left ventricular (LV) diastolic function parameter calc... more The aim of this study was to evaluate the left ventricular (LV) diastolic function parameter calculated using three-dimensional (3D) echocardiography. Using 3D echocardiography and an analysis software program, the left ventricular volume through the cardiac cycle was measured automatically. We therefore calculated 25%, 50%, and 75% of the subtraction end-systolic volume (ESV) from the end-diastolic volume (EDV). The period that the left ventricular volume reached those volumes was calculated from the ESV. Those periods divided all diastolic periods and those calculated values were named D1/4, D1/2, and D3/4, respectively. The peak-filling rate and 1/3 filling fraction (FF) were calculated. E/A, mitral annulus velocities (E&amp;amp;#39;), E/E&amp;amp;#39;, ejection fraction (EF), and left ventricular end-diastolic pressure (LVEDP) were also measured. E/A significantly correlated with D3/4. E/E&amp;amp;#39;significantly correlated with the D1/4, D1/2, and 1/3 filling fraction. E&amp;amp;#39;had a significantly negative correlation with the D1/4, D1/2, D3/4, and 1/3 FF. DT significantly correlated with D1/4, D1/2, D3/4, 1/3 FF, and the mean filling rate and it had a significant negative correlation with the 1/3 filling fraction. EF showed a significant positive correlation with the peak filling ratio. LVEDP demonstrated a significant correlation with D1/4 and D1/2. This study suggests that the LV diastolic indexes as determined by 3D echocardiography provide useful information in the clinical assessment of the diastolic LV function.

Serum levels of oxysterols could be positively correlated with cholesterol absorption and synthesis. However, physiological regulation of various serum oxysterols is largely unknown. The aim of this study was to investigate the relationship between clinical factors and cholesterol metabolism markers, and identify oxysterols associated with cholesterol absorption and synthesis in patients with coronary artery disease. Subjects (n=207) who underwent coronary stenting between 2011 and 2013 were studied cross-sectionally. We measured lipid profiles including serum oxysterols. As for the serum biomarkers of cholesterol synthesis and absorption, oxysterol levels were positively correlated with campesterol and lathosterol. To clarify the influence of BP visit‐to‐visit variability on adverse events in patients with nonvalvular atrial fibrillation, a post hoc analysis of the J‐RHYTHM Registry was performed. Methods and Results Of 7406 outpatients with nonvalvular atrial fibrillation from 158 institutions, 7226 (age, 69.7±9.9 years; men, 70.7%), in whom BP was measured 4 times or more (14.6±5.0 times) during the 2‐year follow‐up period or until occurrence of an event, constituted the study group. SD and coefficient of variation of BP values were calculated as BP variability. Thromboembolism, major hemorrhage, and all‐cause death occurred in 110 (1.5%), 121 (1.7%), and 168 (2.3%) patients, respectively. A consecutive series of outpatients with atrial fibrillation was enrolled from 158 institutions. Of 7937 patients, 7406 with nonvalvular atrial fibrillation (70.8% men, 69.8±10.0 years) were followed for 2 years or until an event occurred. Hypertension was defined as a systolic BP ≥140 mm Hg, a diastolic BP ≥90 mm Hg, a history of hypertension, and/or antihypertensive drug use. Hypertension was an independent risk factor for major hemorrhage (hazard ratio 1.52, 95% CI 1.05-2.21, P=0.027) but not for thromboembolism (hazard ratio 1.05, 95% CI 0.73-1.52, P=0.787). Association between Serum Oxysterols and Coronary Plaque Regression during Lipid-Lowering Therapy with Statin and Ezetimibe: Insights from the CuVIC Trial

Aim: Several clinical trials using intravascular ultrasound (IVUS) evaluation have demonstrated t... more Aim: Several clinical trials using intravascular ultrasound (IVUS) evaluation have demonstrated that intensive lipid-lowering therapy by statin or a combination therapy with statin and ezetimibe results in significant regression of coronary plaque volume. However, it remains unclear whether adding ezetimibe to statin therapy affects coronary plaque composition and the molecular mechanisms of plaque regression. We conducted this prospective IVUS analysis in a subgroup from the CuVIC trial. Methods: The CuVIC trial was a prospective randomized, open, blinded-endpoint trial conducted among 11 cardiovascular centers, where 260 patients with coronary artery disease who received coronary stenting were randomly allocated into either the statin group (S) or the combined statin and ezetimibe group (S E). We enrolled 79 patients (S group, 39 patients; S E group, 40 patients) in this substudy, for whom serial IVUS images of nonculprit lesion were available at both baseline and after 6-8 months of follow-up. Results: After the treatment period, the S E group had significantly lower level of low-density lipoprotein cholesterol (LDL-C; 80.9 3.7 vs. 67.7 3.8 mg/dL, p 0.0143). Campesterol, a marker of cholesterol absorption, and oxysterols (-epoxycholesterol, 4-hydroxycholesterol, and 27-hydroxycholesterol) were also lower in the S E group. IVUS analyses revealed greater plaque regression in the S E group than in the S group (−6.14% vs. −1.18% for each group, p 0.042). It was noteworthy that the lowering of campesterol and 27-hydroxycholesterol, but not LDL-C, had a significant positive correlation with plaque regression. Conclusions: Compared with statin monotherapy, ezetimibe in combination with statin achieved significantly lower LDL-C, campesterol, and 27-hydroxycholesterol, which resulted in greater coronary plaque regression.
