Summary

Allow users to manually rename a plan from the title/header bar above the rendered plan content.

Problem

Plan titles are currently auto-guessed. When the generated title is stale, too generic, or not intuitive to the user, it becomes harder to recognize and find the plan later.

Reproduction steps or desired workflow

1. Generate or open a plan in Warp.
2. Notice the auto-generated plan title in the bar above the actual rendered markdown plan.
3. Choose a rename/edit affordance from that bar.
4. Enter a custom title that is saved with the plan and used anywhere the plan is listed or searched later.
