fixes 2 possible cases for not handling sender error properly:
r.pipesto get deleted. Now, if sender goroutine sends more data the pipe would not be found.fill()gets canceled,update()could get blocked if it manages to fill channel buffer before it itself gets canceled.@AkihiroSuda
