The HMR manager is re-created for each update in the new dev API. This caused `next_hmr_patch_id` to be always `0`. That made the patch file to be always have the same name. This PR fixes that by sharing `next_hmr_patch_id` across all HMR managers. Maybe this was making the tests flaky? refs #5825

Patch file names were not ensured to be unique and caused flaky failures in #5823.
The failure was caused by overriding a patch file with a newer one.

The patch file name generation logic did not generate a unique name because
std::time::SystemTime, which is not monotonic.This PR changes that logic to use a simple counter instead.
