Destructuring a dynamic import namespace binding bailed the export-usage analysis to Complete, so every export was kept even when only a couple were read. Member access on the same binding (ns.a) already tree-shook, so this just brings the destructure path in line.

The trigger is a namespace binding that survives to be destructured, whether it comes from a .then callback param or a non-inlinable await binding.

The fix re-uses the destructure-in-body case through the existing binding handler.

I Added fixtures for the fix and the safety boundaries (rest capture, computed key, multiple accumulating destructures, mixed destructure + member access).

Destructuring a dynamic import namespace binding (`import('mod').then(ns => {
const { a } = ns })`, or the same via a non-inlinable `await` binding) bailed
the export-usage analysis to `Complete`, keeping every export even when only a
few were read. Member access on the same binding (`ns.a`) already tree-shook.

Route the destructure-in-body case through the existing binding-pattern handler
so the named keys are treated like member accesses, reusing its used-key
filtering, rest-binding tracking, and computed-key bail. Re-exported keys
(`export const { a } = ns`) are detected so they are kept even when unread.
Collapse the nine single-purpose dynamic-import destructure fixtures into
two that each fold several behaviors into one entry + multiple libs:

- dynamic_import_body_destructure: the positive cases (`.then` body and a
  non-inlinable `await` binding) covering plain/default/rest destructure,
  member merge, multi-destructure accumulation, and re-export forwarding.
- dynamic_import_body_destructure_bailout: the soundness guards (computed
  key, reassigned binding) that must keep every export.

No coverage lost; the positive fixture still fails without the fix and the
bailout fixture is unchanged with or without it.
