Summary

Add a provider request error classifier for conversation-state rejections from OpenAI, Bedrock/Anthropic-style tool-result mismatches, Gemini role-ordering failures, and alternating-role failures.
Route those failures to a clear message-channel reply instead of silently auto-resetting or falling through to the generic runner failure.
Preserve raw failure details on internal control surfaces and add focused regression coverage for the external-channel behavior.
