The only thing I changed is the "Upgrade" section. I kept all the instructions that you wrote, just cleaned things up a bit and re-ordered.
I wanted to have non-version-specific instructions for parallel and in-place upgrades that we can reuse for any release, and then custom instructions for specific releases as needed.
