This PR updates the MCP configuration handling to expect runtime configuration from NuGet MCP installations, delegating the server.json to runtime config mapping to the chat extension. This pushes the server.json -> runtime config mapping to the chat extension. Code is duplicated to decouple the components, based on @sandy081's recommendation.
