Steps to reproduce:

1. Type hello to Chat.
2. Select the request in the Chat view.
3. Edit hello hello, and resubmit it.
4. .github/instructions/*.instructions.md files are ignored

The log of the first request:
first.copilotmd

The log of the second request:
second.copilotmd

You can see that, in the second request, the file .github/instructions/japanese.instructions.md is ignored.
