Bug Description

In readme, MCP plugin documentation gives 404 as reference is not updated after name change.

Steps to Reproduce

1. Go to home page of repo
2. Browse readme till packages
3. Click on microsoft-teams-mcpplugin
   above link is broken and needs to be updated.

Expected Behavior

By clicking on , it should redirect to mcpplugin readme.

Actual Behavior

By clicking on microsoft-teams-mcpplugin
its giving 404 error as older reference needs to be updated.
