Add a InputFile column to all resultsets in the SQL PerfSnapshot SQL script to distinguish startup vs. shutdown collection

Since Perf snapshot "SQL Server Perf Stats Snapshot.sql" runs at both startup and shutdown, we can add the INPUTFILENAME token  to all resultsets from this file so that we can distinguish whether it came from startup or shutdown files:
