  1. fix: prevent concurrent receive --wait and bound timeout to 120s 

    - Add fs2 file lock to prevent multiple receive --wait per agent (cross-platform)
- Change slash command templates to use --wait --timeout 120 (avoids Claude Code auto-background)
- Add explicit "AT MOST one receive" guidance to all role templates
- Second receive --wait exits immediately with clear error message
