I'd recommend doing the opposite and re-instating the entire Apache license like: #197 |

Yup, it's either this one or that one and I think @brianhelba favors "that". @brianhelba, please close this PR in favor of the other one if that's what we're doing. |

It seems there used to be an Appendix with some advice about how to apply a copyright notice, but there's a parenthesis that still refers to that deleted appendix--this PR strikes the parenthesis.
