Found on Foundry v14, dnd5e v5.3.0
Open up a character sheet and switch to the Features tab. Put your mouse cursor right in the middle of the square box on the right side to expand a feature, showing the activities and description. Instead of expanding the item, it brings up the context menu as if you clicked on the three vertical dots to the right. Either those two icons are just too close together or the area for the vertical dots is overlapping that of the square.
To be extra clear, I'm talking about the "broken square" on the right side of this screenshot. I first noticed this on the Spells tab but it seems to happen on all of them.
Bringing it up with the developer tools, it looks like the three vertical dots button has some padding that overlaps with the square button.
