I won't pretend to have reviewed all of this in detail - I'll let others who are more familiar with the code do that, but I'm very happy to see this PR ready to go in!
One questions about a new IVT though...

@Pilchie we need to access Range and Span types in the compiler to convert back to Roslyn TextSpan and TextLinePosition.

Granting IVT the compiler is a pretty big hammer for that - seems like making those types public might be preferable, but I don't really know the policy for the F# codebase.
