Expose username and password as options for configuration to etcd endpoints.

Originally written by @jipperinbham.

Fixes #1505
Supersedes #1506

Now that --etcd-username and --etcd-password options do not exist,
update description on using client to recommend setting etcd_username
and etcd_password in fleet.conf.

Also fix broken links to etcd v2 documents.

I just fixed broken weblinks to etcd v2 documents, as directory structure of the etcd tree has recently changed.
I'll merge it soon.
