cmdln_macro.h isn't very useful as a public header (nor the other definitions in feat.h and fe.h), there will be a better config interface for 5.0.0

Mostly just remove the redundant ps_ prefix from the pocketsphinx/* header files, should not actually affect the public API which is all accessed through pocketsphinx.h
Also cmdln_macro.h is now hidden, because, hopefully, cmd_ln_t and friends will die soon.
