Previously, messages (tool calls, responses, thinking content) were accumulated in a batch and only flushed to the DB at the end of each agent loop iteration. If cancellation occurred mid-iteration, all accumulated messages were lost. Now each message is persisted to the DB and added to the in-memory conversation immediately when ready. Tool request/response pairs are persisted together to avoid orphaned requests. Also fixes a pre-existing bug where retry logic reset the in-memory conversation but not the DB or client state. Now when retry fires, the DB is synced via replace_conversation and clients are notified via HistoryReplaced. Fixes #7827

AgentEvent::HistoryReplaced is a clean way to notify on rewrite.
the diff highlights the places where there's a possible lost message, like between the tool req/resp pair vs hiding it in a for loop. I like that better.
