This serves both as an example of middleware, and as a useful hint as to how to produce custom error pages (a common request). Fixes aio-libs#888.

Users can specify `max_headers`, `max_line_size`, and `max_field_size` via `app.make_handler()`. Fixes aio-libs#909.
