I thought it was a mistake and I deleted it, but then I realized it wasn't a mistake, and I reversed the change, I'm sorry, that's why those extra commits appear. |

The issue template files were originally in a sub-directory (named "ISSUE_TEMPLATE")
Try to create an issue in your fork (enable it first) and see if it picks up the templates
