All other rules do not have a restriction regarding the Oracle Database version. In the introduction chapter the scope of these guidelines is defined without an explicit Oracle Database version.
As a result for the majority of the rules it is not clear for which Oracle Database version is applicable.
I suggest the following:
Defining the minimal Oracle Database version in the Introduction chapter under the scope for PL/SQL and SQL. I suggest to use Oracle Database 11g Release 2.
Remove restriction in rule G-2230
Rename Restrictions from "ORACLE 12c" to "Oracle Database 12c" or "ORACLE Database 12c". (ORACLE is more than just a database).
Add a Restriction to all guidelines that are not applicable in Oracle Database 11g Release 2. These are for example

maybe more, to be checked according the release information for 4.0
