Added SQLServerTokenIterator class to manually keep track of iterator position and improve performance compared to constantly calling iter.hasNext()

Fixes a variety of problems outlined in #927. Gives users the option to query parameter metadata via USE FMTONLY instead of
sp_describe_undeclared_parameters, which doesn't work in certain scenarios.
