- Please fill out ALL the areas between 1-8 correctly after your surgery.
- If you do not need to change cages on your own, please check 'DCM' box. Alternatively, if there is any special device/food/liquid that needs to be taken care of by you, please check 'Researcher' box.
- Since we do not use any chemical hazardous material, NA should be marked.
- For any viral vector injection please mark accordingly.
