As Eric Dumazet pointed out this also needs to be fixed in IPv6.
v2: Contains the IPv6 tcp/Ipv6 dccp patches as well.

We have seen a few incidents lately where a dst_enty has been freed
with a dangling TCP socket reference (sk->sk_dst_cache) pointing to that
dst_entry. If the conditions/timings are right a crash then ensues when the
freed dst_entry is referenced later on. A Common crashing back trace is:

It's found the freed dst_entry here:

But there are other backtraces attributed to the same freed dst_entry in
netfilter code as well.

All the vmcores showed 2 significant clues:

- Remote hosts behind the default gateway had always been redirected to a
different gateway. A rtable/dst_entry will be added for that host. Making
more dst_entrys with lower reference counts. Making this more probable.

- All vmcores showed a postitive LockDroppedIcmps value, e.g:

LockDroppedIcmps                  267

A closer look at the tcp_v4_err() handler revealed that do_redirect() will run
regardless of whether user space has the socket locked. This can result in a
race condition where the same dst_entry cached in sk->sk_dst_entry can be
decremented twice for the same socket via:

do_redirect()->__sk_dst_check()-> dst_release().

Which leads to the dst_entry being prematurely freed with another socket
pointing to it via sk->sk_dst_cache and a subsequent crash.

To fix this skip do_redirect() if usespace has the socket locked. Instead let
the redirect take place later when user space does not have the socket
locked.

The dccp/IPv6 code is very similar in this respect, so fixing it there too.

As Eric Garver pointed out the following commit now invalidates routes. Which
can set the dst->obsolete flag so that ipv4_dst_check() returns null and
triggers the dst_release().

mipsxx_pmu_handle_shared_irq() calls irq_work_run() while holding the
pmuint_rwlock for read.  irq_work_run() can, via perf_pending_event(),
call try_to_wake_up() which can try to take rq->lock.

However, perf can also call perf_pmu_enable() (and thus take the
pmuint_rwlock for write) while holding the rq->lock, from
finish_task_switch() via perf_event_context_sched_in().

This leads to an ABBA deadlock:

The lock seems to be there to protect the hardware counters so there is
no need to hold it across irq_work_run().

We miss a spin_lock_init() in igmpv3_add_delrec(), probably
because previously we never use it on this code path. Since
we already unlink it from the global mc_tomb list, it is
probably safe not to acquire this spinlock here. It does not
harm to have it although, to avoid conditional locking.

A vendor with a system having more than 128 CPUs occasionally encounters
the following crash during shutdown. This is not an easily reproduceable
event, but the vendor was able to provide the following analysis of the
crash, which exhibits the same footprint each time.

This was first introduced in 7ea0ed2 ipmi: Make the
message handler easier to use for SMI interfaces
where some code was moved outside of the rcu_read_lock()
and the lock was not added.

The dummy-hcd driver calls the gadget driver's disconnect callback
under the wrong conditions.  It should invoke the callback when Vbus
power is turned off, but instead it does so when the D+ pullup is
turned off.

This can cause a deadlock in the composite core when a gadget driver

It is only when one asks for checking what of those entries actually had
samples, i.e. when we use either -H or --with-hits, that we will process
all the PERF_RECORD_ events, and since tools/perf/builtin-buildid-list.c
neither explicitely set a perf_tool.namespaces() callback nor the
default stub was set that we end up, when processing a
PERF_RECORD_NAMESPACE record, causing a SEGFAULT:

As it turns out, this is because the i_itemp pointer, along
with the d_ops pointer, has been overwritten with zeros
when we tear down the extents during truncate.  When the in-core
inode fork on the temporary inode used by xfs_fsr was originally
set up during the extent swap, we mistakenly looked at di_nextents
to determine whether all extents fit inline, but this misses extents
generated by speculative preallocation; we should be using if_bytes
instead.

This mistake corrupts the in-memory inode, and code in
xfs_iext_remove_inline eventually gets bad inputs, causing
it to memmove and memset incorrect ranges; this became apparent
because the two values in ifp->if_u2.if_inline_ext[1] contained
what should have been in d_ops and i_itemp; they were memmoved due
to incorrect array indexing and then the original locations
were zeroed with memset, again due to an array overrun.

Fix this by properly using i_df.if_bytes to determine the number
of extents, not di_nextents.

Thanks to dchinner for looking at this with me and spotting the
root cause.

We miss a spin_lock_init() in igmpv3_add_delrec(), probably
because previously we never use it on this code path. Since
we already unlink it from the global mc_tomb list, it is
probably safe not to acquire this spinlock here. It does not
harm to have it although, to avoid conditional locking.

A vendor with a system having more than 128 CPUs occasionally encounters
the following crash during shutdown. This is not an easily reproduceable
event, but the vendor was able to provide the following analysis of the

The dummy-hcd driver calls the gadget driver's disconnect callback
under the wrong conditions.  It should invoke the callback when Vbus
power is turned off, but instead it does so when the D+ pullup is
turned off.

This can cause a deadlock in the composite core when a gadget driver

On some platforms(e.g. rk3399 board), we can call hcd_add/remove
consecutively without calling usb_put_hcd/usb_create_hcd in between,
so hcd->flags can be stale.

If the HC dies due to whatever reason then without this patch we get
the below error on next hcd_add.

Dmitry reported a lockdep splat [1] (false positive) that we can fix
by releasing the spinlock before calling icmp_send() from ip_expire()

This is a false positive because sending an ICMP message can not

If an OVS_ATTR_NESTED attribute type is found while walking
through netlink attributes, we call nlattr_set() recursively
passing the length table for the following nested attributes, if
different from the current one.

However, once we're done with those sub-nested attributes, we
should continue walking through attributes using the current
table, instead of using the one related to the sub-nested
attributes.

For example, given this sequence:

It turns out that a NULL pointer is referenced. Here are the
function calls:

Function parse_events(xx, xx, struct parse_events_error *err) dives
into a bison generated scanner and creates
parser state information for it first:

Now various functions inside the bison scanner are called to end up in
__parse_events_add_pmu(struct parse_events_state *parse_state, ..) with
first parameter being a pointer to above structure definition.

Now the PMU event name is not found (because being executed in a VM) and
this function tries to create an error message with

   asprintf(&parse_state->error.str, ....)

which references a NULL pointer and dumps core.

Fix this by providing a pointer to the necessary error information
instead of NULL. Technically only the else part is needed to avoid the
core dump, just lets be safe...

This task was trying to mount the cdrom.  It allocated and configured a
super_block struct and owned the write-lock for the super_block->s_umount

This task tried to open the cdrom device, the sr_block_open function
acquired the global sr_mutex lock. The call to check_disk_change()
then saw an event flag indicating a possible media change and tried
to flush any cached data for the device.
As part of the flush, it tried to acquire the super_block->s_umount
lock associated with the cdrom device.
This was the same super_block as created and locked by the previous task.

The first task acquires the s_umount lock and then the sr_mutex_lock;
the second task acquires the sr_mutex_lock and then the s_umount lock.

This patch fixes the issue by moving check_disk_change() out of
cdrom_open() and let the caller take care of it.

This logic and its ipv4 counterpart read the destination port from
the packet at skb_transport_offset(skb) + 4.

With MSG_MORE and a local SOCK_RAW sender, syzbot was able to cook a
packet that stores headers exactly up to skb_transport_offset(skb) in
the head and the remainder in a frag.

Call pskb_may_pull before accessing the pointer to ensure that it lies
in skb head.

It happens in the pagefault and results in double pagefault
during compacting pages when memory allocation fails.

Analysed the vmcore, the page leads to second pagefault is corrupted
with _mapcount=-256, but private=0.

It's caused by the race between migration and ballooning, and lock
missing in virtballoon_migratepage() of virtio_balloon driver.
This patch fix the bug.

Currently we have structrues comp (which is empty) and comp_info being
used to register and deregister the component.  This mismatch in naming
occurred from a previous commit that renamed aim_info to comp. Fix this
to use consistent component naming in line with most/net, most/sound etc.

This fixes the message two issues, one with a null empty name when
loading the module:

The locking order of vlan_rwsem (LOCK A) and then rtnl (LOCK B),
contradicts other flows such as ipoib_open possibly causing a deadlock.
To prevent this deadlock heavy flush is called with RTNL locked and
only then tries to acquire vlan_rwsem.

It was observed that a process blocked indefintely in
__fscache_read_or_alloc_page(), waiting for FSCACHE_COOKIE_LOOKING_UP
to be cleared via fscache_wait_for_deferred_lookup().

At this time, ->backing_objects was empty, which would normaly prevent
__fscache_read_or_alloc_page() from getting to the point of waiting.
This implies that ->backing_objects was cleared *after*
__fscache_read_or_alloc_page was was entered.

When an object is "killed" and then "dropped",
FSCACHE_COOKIE_LOOKING_UP is cleared in fscache_lookup_failure(), then
KILL_OBJECT and DROP_OBJECT are "called" and only in DROP_OBJECT is
->backing_objects cleared.  This leaves a window where
something else can set FSCACHE_COOKIE_LOOKING_UP and
__fscache_read_or_alloc_page() can start waiting, before
->backing_objects is cleared

There is some uncertainty in this analysis, but it seems to be fit the
observations.  Adding the wake in this patch will be handled correctly
by __fscache_read_or_alloc_page(), as it checks if ->backing_objects
is empty again, after waiting.

Customer which reported the hang, also report that the hang cannot be
reproduced with this fix.

info->nr_rings isn't adjusted in case of ENOMEM error from
negotiate_mq(). This leads to kernel panic in error path.

When we hit an I/O error in free_log_tree->walk_log_tree during file system
shutdown we can crash due to there not being a valid transaction handle.

Use btrfs_handle_fs_error when there's no transaction handle to use.

Seeing as we use this registermap in the context of our IRQ handlers, we
need to be using spinlocks for reading/writing registers so that we can
still read them from IRQ handlers without having to grab any mutexes and

The *_frag_reasm() functions are susceptible to miscalculating the byte
count of packet fragments in case the truesize of a head buffer changes.
The truesize member may be changed by the call to skb_unclone(), leaving
the fragment memory limit counter unbalanced even if all fragments are
processed. This miscalculation goes unnoticed as long as the network
namespace which holds the counter is not destroyed.

Should an attempt be made to destroy a network namespace that holds an
unbalanced fragment memory limit counter the cleanup of the namespace
never finishes. The thread handling the cleanup gets stuck in
inet_frags_exit_net() waiting for the percpu counter to reach zero. The
thread is usually in running state with a stacktrace similar to:

It is not possible to create new network namespaces, and processes
that call unshare() end up being stuck in uninterruptible sleep state
waiting to acquire the net_mutex.

The bug was observed in the IPv6 netfilter code by Per Sundstrom.
I thank him for his analysis of the problem. The parts of this patch
that apply to IPv4 and IPv6 fragment reassembly are preemptive measures.

It was observed that a process blocked indefintely in
__fscache_read_or_alloc_page(), waiting for FSCACHE_COOKIE_LOOKING_UP
to be cleared via fscache_wait_for_deferred_lookup().

At this time, ->backing_objects was empty, which would normaly prevent
__fscache_read_or_alloc_page() from getting to the point of waiting.
This implies that ->backing_objects was cleared *after*
__fscache_read_or_alloc_page was was entered.

When an object is "killed" and then "dropped",
FSCACHE_COOKIE_LOOKING_UP is cleared in fscache_lookup_failure(), then
KILL_OBJECT and DROP_OBJECT are "called" and only in DROP_OBJECT is
->backing_objects cleared.  This leaves a window where
something else can set FSCACHE_COOKIE_LOOKING_UP and
__fscache_read_or_alloc_page() can start waiting, before
->backing_objects is cleared

There is some uncertainty in this analysis, but it seems to be fit the
observations.  Adding the wake in this patch will be handled correctly
by __fscache_read_or_alloc_page(), as it checks if ->backing_objects
is empty again, after waiting.

Customer which reported the hang, also report that the hang cannot be
reproduced with this fix.

So, fix this by enabling the fast_io option in our regmap config so that

The *_frag_reasm() functions are susceptible to miscalculating the byte
count of packet fragments in case the truesize of a head buffer changes.
The truesize member may be changed by the call to skb_unclone(), leaving
the fragment memory limit counter unbalanced even if all fragments are
processed. This miscalculation goes unnoticed as long as the network
namespace which holds the counter is not destroyed.

Should an attempt be made to destroy a network namespace that holds an
unbalanced fragment memory limit counter the cleanup of the namespace
never finishes. The thread handling the cleanup gets stuck in
inet_frags_exit_net() waiting for the percpu counter to reach zero. The
thread is usually in running state with a stacktrace similar to:

It is not possible to create new network namespaces, and processes
that call unshare() end up being stuck in uninterruptible sleep state
waiting to acquire the net_mutex.

The bug was observed in the IPv6 netfilter code by Per Sundstrom.
I thank him for his analysis of the problem. The parts of this patch
that apply to IPv4 and IPv6 fragment reassembly are preemptive measures.

It was observed that a process blocked indefintely in
__fscache_read_or_alloc_page(), waiting for FSCACHE_COOKIE_LOOKING_UP
to be cleared via fscache_wait_for_deferred_lookup().

At this time, ->backing_objects was empty, which would normaly prevent
__fscache_read_or_alloc_page() from getting to the point of waiting.
This implies that ->backing_objects was cleared *after*
__fscache_read_or_alloc_page was was entered.

When an object is "killed" and then "dropped",
FSCACHE_COOKIE_LOOKING_UP is cleared in fscache_lookup_failure(), then
KILL_OBJECT and DROP_OBJECT are "called" and only in DROP_OBJECT is
->backing_objects cleared.  This leaves a window where
something else can set FSCACHE_COOKIE_LOOKING_UP and
__fscache_read_or_alloc_page() can start waiting, before
->backing_objects is cleared

There is some uncertainty in this analysis, but it seems to be fit the
observations.  Adding the wake in this patch will be handled correctly
by __fscache_read_or_alloc_page(), as it checks if ->backing_objects
is empty again, after waiting.

Customer which reported the hang, also report that the hang cannot be
reproduced with this fix.

Commit 9b6f7e1 ("mm: rework memcg kernel stack accounting") will
result in fork failing if allocating a kernel stack for a task in
dup_task_struct exceeds the kernel memory allowance for that cgroup.

Unfortunately, it also results in a crash.

This is due to the code jumping to free_stack and calling
free_thread_stack when the memcg kernel stack charge fails, but without
tsk->stack pointing at the freshly allocated stack.

This in turn results in the vfree_atomic in free_thread_stack oopsing
with a backtrace like this:

Function ib_create_qp() was failing to return an error when
rdma_rw_init_mrs() fails, causing a crash further down in ib_create_qp()

As Eric Dumazet pointed out this also needs to be fixed in IPv6.
v2: Contains the IPv6 tcp/Ipv6 dccp patches as well.

We have seen a few incidents lately where a dst_enty has been freed
with a dangling TCP socket reference (sk->sk_dst_cache) pointing to that
dst_entry. If the conditions/timings are right a crash then ensues when the
freed dst_entry is referenced later on. A Common crashing back trace is:

But there are other backtraces attributed to the same freed dst_entry in
netfilter code as well.

All the vmcores showed 2 significant clues:

- Remote hosts behind the default gateway had always been redirected to a
different gateway. A rtable/dst_entry will be added for that host. Making
more dst_entrys with lower reference counts. Making this more probable.

- All vmcores showed a postitive LockDroppedIcmps value, e.g:

LockDroppedIcmps                  267

A closer look at the tcp_v4_err() handler revealed that do_redirect() will run
regardless of whether user space has the socket locked. This can result in a
race condition where the same dst_entry cached in sk->sk_dst_entry can be
decremented twice for the same socket via:

do_redirect()->__sk_dst_check()-> dst_release().

Which leads to the dst_entry being prematurely freed with another socket
pointing to it via sk->sk_dst_cache and a subsequent crash.

To fix this skip do_redirect() if usespace has the socket locked. Instead let
the redirect take place later when user space does not have the socket
locked.

The dccp/IPv6 code is very similar in this respect, so fixing it there too.

As Eric Garver pointed out the following commit now invalidates routes. Which
can set the dst->obsolete flag so that ipv4_dst_check() returns null and
triggers the dst_release().

Commit 1fa5efe (ext4: Use generic helpers for quotaon
and quotaoff) made possible to call quotactl(Q_XQUOTAON/OFF) on ext4 filesystems
with sysfile quota support. This leads to calling dquot_enable/disable without s_umount
held in excl. mode, because quotactl_cmd_onoff checks only for Q_QUOTAON/OFF.

This patch locks the superblock ->s_umount sem. in exclusive mode for all Q_XQUOTAON/OFF
quotactls too in addition to Q_QUOTAON/OFF.

AFAICT, other than ext4, only xfs and ocfs2 are affected by this change.
The VFS will now call in xfs_quota_* functions with s_umount held, which wasn't the case
before. This looks good to me but I can not say for sure. Ext4 and ocfs2 where already
beeing called with s_umount exclusive via quota_quotaon/off which is basically the same.

This patch fixes NULL pointer crash due to active timer running for abort
IOCB.

From crash dump analysis it was discoverd that get_next_timer_interrupt()

If, for any reason, userland shuts down iscsi transport interfaces
before proper logouts - like when logging in to LUNs manually, without
logging out on server shutdown, or when automated scripts can't
umount/logout from logged LUNs - kernel will hang forever on its
sd_sync_cache() logic, after issuing the SYNCHRONIZE_CACHE cmd to all
still existent paths.

This happens because iscsi_eh_cmd_timed_out(), the transport layer
timeout helper, would tell the queue timeout function (scsi_times_out)
to reset the request timer over and over, until the session state is
back to logged in state. Unfortunately, during server shutdown, this
might never happen again.

Other option would be "not to handle" the issue in the transport
layer. That would trigger the error handler logic, which would also need
the session state to be logged in again.

Best option, for such case, is to tell upper layers that the command was
handled during the transport layer error handler helper, marking it as
DID_NO_CONNECT, which will allow completion and inform about the
problem.

After the session was marked as ISCSI_STATE_FAILED, due to the first
timeout during the server shutdown phase, all subsequent cmds will fail
to be queued, allowing upper logic to fail faster.

Check for that and return NULL instead.

This problem was introduced recently, the other codepaths leading to
thread_trace__files_entry() check for negative fds, like thread__fd_path(),
