Add log warning in AgentJarIndex if an entryKey has multiple prefixIds. (#8434)

* Add log warning if entryKey has multiple prefixIds.

* Only warn if prefixId changes.

* Improve log warning to describe the situation instead of the state.

* Update log warning message.
