This field is related to honeypot spam protection, which comes with Formidable Forms. It should not be visible. If you see this field at the top of your form, try the following steps to resolve your issue.

1. Make sure WordPress and Formidable Forms are fully up to date.
2. Go to your Formidable → Styles page and click the Update button to refresh the styling.
3. If you have a plugin or theme that minifies CSS, try turning off the minification temporarily.
4. Clear all caching mechanisms on your site (plugins, hosting caches, etc.) and revisit your form.
5. Clear your browser's cache.
6. Add the following to your Formidable → Global Settings → Custom CSS box:

   .frm_forms input[type=text].frm_verify{
       display:none !important;
   }
