To overview the recent literature on the use of MRI and musculoskeletal ultrasonography (MSUS) in... more To overview the recent literature on the use of MRI and musculoskeletal ultrasonography (MSUS) in rheumatoid arthritis. Subclinical inflammation has been widely confirmed, even in the earliest phases of rheumatoid arthritis. The presence of osteitis has added benefits to modern diagnostic criteria, and anticitrullinated peptide antibody positive patients have demonstrated higher osteitis scores. A model for prediction of rheumatoid arthritis onset employing usual clinical data and power Doppler ultrasonography has been reported. The presence of tenosynovitis may also be an early finding in rheumatoid arthritis. Modern imaging continues to inform our concept of pathogenesis with reports on the direct relationship of synovitis to cartilage proteoglycan loss using compositional MRI measures. Growing data on the validity of MRI as an important predictor of clinical and radiographic damage endpoints has been reported and reflected in the growing use of this outcome in many contemporary biologic therapy trials. Much work has been presented on improved and validated MSUS scores with reduced and feasible joint counts. The role of ultrasonography in making sensible decisions when monitoring biologic use, and in tapering, has been reported. The recent literature demonstrates improved validity and utility for both MRI and MSUS in diagnosis, prognosis and monitoring of rheumatoid arthritis.

Rheumatoid arthritis predisposes to osteoporosis and most physicians supplement calcium and 1,25 ... more Rheumatoid arthritis predisposes to osteoporosis and most physicians supplement calcium and 1,25 dihydroxy vitamin D3 along with the standard treatment with disease modifying drugs. Advances in the understanding of physiological actions of 1,25 dihydroxy vitamin D3 suggest that this molecule plays an important role in modulating the immune response of the body. Vitamin D receptor has been found in many cells of the immune system including promyelocytes, myelocytes and macrophages. Experiments in animal models of rheumatoid arthritis have shown amelioration of the disease after administration of 1,25 dihydroxy vitamin D3. This article reviews the available data regarding the role of 1,25 dihydoxy vitamin D3 in rheumatoid arthritis.

Childhood-onset Takayasu arteritis (c-TA) is a distinct subset affecting a wide age group, rangin... more Childhood-onset Takayasu arteritis (c-TA) is a distinct subset affecting a wide age group, ranging from young infants to adolescents and it differs from adult TA in many aspects. There is scarcity of data on c-TA worldwide. The disease is classified using the European League Against Rheumatism/Pediatric Rheumatology International Trials Organization/Pediatric Rheumatology European Society criteria. The non-specific nature of presenting complaints and lack of appropriate biomarkers delay the early diagnosis of this illness and many children present with complications, which become irreversible once they set in. One of the largest cohorts of 40 children with c-TA from our center reports hypertension as the commonest presenting feature. Systemic symptoms like headache, fever and weight loss are also described. Assessment of disease in c-TA is done by correlating clinical features with raised inflammatory markers. Advanced imaging plays an important role in diagnosis. In c-TA, the role of magnetic resonance angiography is advocated, taking into consideration the enormous amount of radiation exposure with other modalities. Complications of c-TA include cardiovascular, pulmonary, neurological and those arising secondary to long-term steroid and immunosuppression therapy.

AIM: To study the clinical presentation of young onset gout in our tertiary care teaching hospita... more AIM: To study the clinical presentation of young onset gout in our tertiary care teaching hospital. METHODS: Case records of 117 patients of gout fulfilling the ACR criteria from our centre over the last 4 years were retrieved. Patients who had onset of disease at or below the age of 30 were called young onset gout (YOG). Clinical and laboratory parameters of this subset of patients were compared with those who had onset of gout above the age of 30 years. RESULTS: Of the 117 patients with gout, 21 (17.95%) fulfilled the criteria of YOG. The mean age at the onset of gout was 23.67 years (range, 17-30) in the YOG subset and 40.04 years (range, 31-69) in the other group. The male: female ratio was 20:1 in the YOG group as compared to 18.2:1 in the other group. Clinically, acute intermittent polyarthritis was observed in 19 patients with YOG. In this subset Podagra alone was observed in 1 patient as compared to 29 patients in the other. Tophi observed in 8 patients with YOG developed over a mean period of 2.63 years as compared to 17 patients (17.7%) in the other group who developed the same over a mean time of 7.12 years. CONCLUSION: Young onset gout comprised about 18% of all gout patients. This subset has a different clinical presentation; acute intermittent polyarthritis, early tophi formation and paucity of Podagra, as compared to patients with onset of gout at more than 30 years of age.

Enthesopathy is a common clinical finding denoting pathology at the &... more Enthesopathy is a common clinical finding denoting pathology at the 'entheses', i.e. attachment sites of muscles, tendons, joint capsules, ligaments and fascia to the bone. Inflammatory enthesopathy or enthesitis is a sine qua non of seronegative spondyloarthropathies (SSA). It can also be occupational, metabolic, drug induced, infective or degenerative. Bursitis closely mimics enthesitis. Ultrasound with high frequency transducers is a simple, cost-effective and feasible test to detect enthesopathy which is amenable to treatment with local steroid injections, physiotherapy and non-steroidal anti-inflammatory drugs, in addition to treatment of the primary disease. Unrecognized and untreated, it can lead to considerable morbidity.

After oral administration of antipyrine (15 mg/kg), salivary concentration of the drug was measured at various time intervals by high performance liquid chromatography method. In type I diabetics the clearance rate (CL) and apparent volume of distribution (V) of the drug were significantly higher when compared to corresponding controls. The elimination half-life (t 1/2) remained unaltered. In type II diabetics, the t 1/2 of the drug was increased due to increase in V. There was no significant difference in CL. It is concluded that antipyrine elimination is enhanced in type I diabetics while in type II patients it is unaltered.

Kikuchi's disease is a histologically alarming self-limiting condition typically ... more Kikuchi's disease is a histologically alarming self-limiting condition typically affecting the lymph nodes of young females. A 13-year-old girl was presented with fever, skin rash and cervical lymphadenopathy. On examination she was found febrile, mild pallor was present and she had lymphadenopathy. Liver was palpable. Cervical lymph node biopsy showed histiocytic necrotising lymphadenitis (Kikuchi's disease). Dengue virus serology for IgG blot showed evidence of seroconversion in serial samples. She was treated with antibiotics and fluconazole and cyclosporin A. During hospitalisation she developed retinal vasculitis. She was reviewed after one month and showed rashes of subacute cutaneous lupus erythematosus. This case can be described to be a triggering event by dengue viral infection causing abnormal immune response leading initially to Kikuchi's disease and later on to systemic lupus erythematosus.

Polymyositis is a rare autoimmune manifestation of chronic graft-versus-host disease (GVHD) chara... more Polymyositis is a rare autoimmune manifestation of chronic graft-versus-host disease (GVHD) characterized by muscle pain, weakness, and an increase in muscle-related enzymes that responds well to treatment with immunosuppressive agents such as steroids and cyclosporine. We describe a case in which polymyositis was the main manifestation of chronic GVHD that occurred 12 months after allogeneic bone marrow transplantation in a patient with acute lymphocytic leukemia (ALL). The polymyositis responded well to treatment with steroids and cyclosporine, with no relapse of symptoms on tapering of the medication.

Neuropsychiatric abnormalities frequently occur in patients with systemic lupus erythematosus, af... more Neuropsychiatric abnormalities frequently occur in patients with systemic lupus erythematosus, affecting as many as 14-75% of people with this disease. High-dose steroid with or without anticoagulation is the mainstay of treatment in neuropsychiatric systemic lupus erythematosus (NPSLE). Use of mycophenolate as a steroid sparing drug may be a potential alternative agent in the therapy of NPLE, but lack of randomized trials and cost prohibit its widespread use. Its safety profile is higher than that of cyclophosphamide and azathioprine. We report a successfully treated case of neuropsychiatric systemic lupus erythematosus, presenting as psychosis, whose longterm remission was maintained on treatment with mycophenolate mofetil.
