Dr. Ece Yamak Altinpulluk received her MD degree from Istanbul University Cerrahpasa Medical Faculty, Istanbul, Turkey in 2003. She took Training period at the Department of Cell Biology and Genetics, Erasmus Medical Faculty, Rotterdam, the Netherlands in 2002. Between 2006-2007, She made Internal Medicine Residency At Gazi Medical Faculty, Ankara Turkey. She completed her anesthesiology and reanimation resident training at Istanbul University Cerrahpasa Medical Faculty, Istanbul, Turkey from 2009 to 2013. She was be assigned in Trakya University Medical University, Edirne Turkey from 2014 to 2015. Since 2015 She has been working as Anesthesiologist at Istanbul University Cerrahpasa Medical Faculty, Istanbul, Turkey. In 2016, she worked as a fellow in Regional Anesthesia at Hospital Universitario Mostoles Department of Anesthesiology, Madrid, Spain. In 20017 She has been as observer doctor at Acute Pain Management Service in Cleveland Clinic, Anesthesiology Institute, Department of General Anesthesia, Cleveland, OH. She has been invited as an instructor to the European Society of Regional Anesthesia Workshops, Madrid in 2017 and 2018 and She is a member of Ultradissection Spain EchoTraining School. She was awarded a grant by The Scientific and Technological Research Council of Turkey (TUBITAK) in 2018 (International Postdoctoral Research Fellowship Programme in Cleveland Clinic, USA) and has started to work as Research Fellow at Department of Outcomes Research in Cleveland Clinic, the USA since April 2019. N-methyl-D-aspartate (NMDA) receptor antagonists have a well-established role in the development and modulation of chronic neuropathic pain. Nitrous oxide is widely used and generally safe anesthetic gas with NMDA receptor antagonist activity. We therefore tested the hypothesis that brief periods of nitrous oxide exposure reduce pain in patients with CRPS.MethodsPatients with a diagnosis of CRPS were randomized to either 2 hours of nitrous oxide exposure on three alternating days (Nitrous Oxide) versus a placebo air/oxygen mixture (Air-Oxygen). Only around 100 cases have been reported in the literature. Herein, we present a case with a recurrent ovarian cancer who underwent secondary cytoreductive surgery and hyperthermic intraperitoneal chemotherapy. An oval defect with dimensions of 3 × 4 cm was seen in the left posterolateral site of the diaphragm during surgical exploration. In addition, a 6 × 3 cm iatrogenic right-sided diaphragmatic defect was found and repaired. In the early postoperative period, a bilateral thalamic infarction occurred.

Regional anesthesia has a very interesting long history, initially preferred over general anesthe... more Regional anesthesia has a very interesting long history, initially preferred over general anesthesia because of safety concerns, then for a period general anesthesia became safer and was preferred. The use of innovative technologies such as ultrasound technology has made the blocks safer and successful by directly visualizing targeted nerves and the location of local anesthetics. With the wide use of ultrasound in the regional anesthesia field success rate of peripheral nerve blocks increased and novel blocks techniques developed. Moreover, new extended-release local anesthetic agents have begun to be promising timeefficient and longer duration of analgesia with a single injection. In this article, we attempt to summarize some of the novel block techniques, pharmacological agents, and new technologies in the field of regional anesthesia.

Resumen La cirugia de correccion de la columna es un procedimiento quirurgico invasivo que origin... more Resumen La cirugia de correccion de la columna es un procedimiento quirurgico invasivo que origina dano postoperatorio severo. Reportamos 6 casos de bloqueo en el plano del musculo erector de la columna para cirugia de escoliosis. Nuestro objetivo fue aportar tratamiento efectivo para el dolor perioperatorio, y lograr estabilidad hemodinamica intraoperatoria, sin interferir en la neuromonitorizacion. Tambien se destacan los retos tecnicos. El escaner ecoguiado es necesario para identificar las prominencias oseas y determinar los posibles puntos de inyeccion multiples. El bloqueo en el plano del musculo erector de la columna en cirugia de escoliosis es una tecnica mas facil y segura, en comparacion con la anestesia epidural, pudiendo utilizar cirugia de columna compleja instrumentada. Este bloqueo parece desempenar un papel en la via del dolor perioperatorio, complementando el regimen anestesico multimodal y no interfiriendo en los potenciales evocados en adultos. Sin embargo, el mecanismo de difusion de este bloqueo es poco conocido aun y, por tanto, debemos ser conscientes de la toxicidad del anestesico local.

Viscosupplementation by hyaluronic acid (HA) is well established non-surgical treatment of knee o... more Viscosupplementation by hyaluronic acid (HA) is well established non-surgical treatment of knee osteoarthritis (KOA). This registry-based study investigated the booster effect of a quarterly intra-articular single knee injection (30mg/2ml) for five years. Methods Sixty patients, including 29 males and 31 females, with a mean ± SD age 61.07 ± 9.15 with Kellgren-Lawrence grade I-III KOA, have been selected from a registry of interventional treatments for musculoskeletal pain conditions. To be eligible, patients had to be treated with a single quarterly intraarticular injection of HA with a follow-up of at least five years and assessed with Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Numeric Rating Scale (NRS) at baseline and after each HA injection in the first 24 months and at 36, 48, and 60 months. Results Sixty of 63 patients enrolled in this study completed the 60 months of follow-up. Patients had a marked improvement in knee function and pain, expressed by the significant reduction in WOMAC (T0 48.62±8.95 vs. T11 10.75±4.36; p<0.0001) and NRS scores (6.38±1.06 vs. T11 0.95±0.89 p<0.0001) from the baseline to the end of the follow-up period. Conclusion A quarterly injection of HA provides a rapid, safe, and stable long-term reduction of pain and improvement of function in elderly people with mild to moderate knee osteoarthritis along a five-year period of treatment and follow-up. Further investigations are necessary to confirm these findings.

The ultrasound-guided erector spinae plane block has been orginally described for providing thoracic analgesia at the T5 transverse process by Forero et al. We performed post-operative bilateral erector spinae plane blocks with 20 ml bupivacaine 0.25% at the level of the T9 transverse process in a pregnant woman after caesarean section. In this report, we described that bilateral erector spinae plane block at T9 level provides effective and long-lasting postoperative analgesia for lower abdominal surgery.

Methods: Seventy-five consecutive patients with painful shoulder were treated with the SHAC, alone (30 patients) or in combination with a suprascapular nerve block (SSnb: 25 patients) or with pectoralis and serratus plane block (PECS-2: 20 patients). All blocks were performed with 0.2% ropivacaine plus 8 mg dexamethasone. All patients were treated with threeweekly physiotherapy sessions for the following 2 weeks and then with home exercises. Results: The post-procedural analgesic effect was strong in all groups, with a mean change in numeric rating scale (NRS) values of-6.05 in group 1,-6.25 in group 2, and-6.19 in group 3 (p \ .0001), allowing all patients to complete an immediate physiotherapy session. We report six cases in which Bilateral Erector Spinae Plane Block was performed for scoliosis surgery. Our aim was to provide an effective perioperative pain management and to achieve intraoperative hemodynamic stability with no interference on neuromonitoring. The technical challenges are also highlighted. An ultrasound guided scout scan is necessarry to identify the bony prominences and determine the possible multiple injection points. Erector Spinae Plane Block in scoliosis surgery is an easier and safer technique compared to epidural anesthesia and can use instrumented complex spinal surgery. This block seems to have a role in perioperative pain pathway complementing the multimodal analgesic regimen and not have interference with evocated potentials in adults. However the diffusion mechanism of the this block is not well known hence it should be awake regarding local anesthetic toxicity.
