IMPORTANCE Although myopic maculopathy has become a major cause of vision impairment worldwide, f... more IMPORTANCE Although myopic maculopathy has become a major cause of vision impairment worldwide, few data from Russia and Central Asia on the prevalence of myopic maculopathy have been available. OBJECTIVE To assess the prevalence of myopic maculopathy and its associations with ocular and systemic parameters in a population in Russia. DESIGN, SETTING, AND PARTICIPANTS The Ural Eye and Medical Study, a population-based casecontrol study, was conducted in rural and urban areas in Bashkortostan, Russia, from October 26, 2015, to July 4, 2017. Data analysis was performed from September 13 to September 15, 2019. The Ural Eye and Medical Study included 5899 of 7328 eligible individuals (80.5%) aged 40 years or older. EXPOSURES A detailed ocular and systemic examination included fundus photography and optic coherence tomography for the assessment of myopic maculopathy. MAIN OUTCOMES AND MEASURES Prevalence of myopic maculopathy. RESULTS The present investigation included 5794 of the 5899 eligible individuals (98.2%; 3277 [56.6%] women; mean [SD] age, 58.9 [10.7] years) with available information about myopic maculopathy. Mean (SD) axial length was 23.3 (1.1) mm (range, 19.78-32.87 mm). Prevalence of any myopic maculopathy was 1.3% (95% CI, 1.0%-1.6%); myopic maculopathy stage 2, 0.8% (95% CI, 0.6%-10.0%); stage 3, 0.2% (95% CI, 0.1%-0.4%); and stage 4, 0.2% (95% CI, 0.1%-0.4%). The prevalence of moderate to severe vision impairment and blindness was 29.8% (14 of 47 participants; 95% CI, 16.2%-43.3%) in stage 2 myopic maculopathy, 57.1% (8 of 14 participants; 95% CI, 27.5%-86.8%) in stage 3, and 100% (13 of 13 participants; 95% CI, 100%-100%) in stage 4. In multivariable analysis, a higher myopic maculopathy prevalence was associated with longer axial length (odds ratio [OR], 4.54; 95% CI, 3.48-5.92; P < .001), older age (OR, 1.04; 95% CI, 1.01-1.07; P = .03), and thinner peripapillary retinal nerve fiber layer thickness (OR, 0.96; 95% CI, 0.95-0.98; P < .001). After exclusion of glaucomatous eyes, the association between myopic maculopathy prevalence and thinner retinal nerve fiber layer remained significant (OR, 0.96; 95% CI, 0.95-0.98; P < .001). Myopic maculopathy prevalence was not significantly associated with sex; region of habitation; level of education; ethnicity; prevalence of arterial hypertension, chronic obstructive pulmonary disease, chronic kidney disease, diabetes, and inflammatory liver disease; hearing loss; depression score; or anxiety score. CONCLUSIONS AND RELEVANCE In this ethnically mixed population from Russia, myopic maculopathy prevalence was mainly associated with elongated axial length and thinner peripapillary (continued)
Цель-оценка уровня рибофлавина во влаге передней камеры (вПК) глаза экспериментальных животных в ... more Цель-оценка уровня рибофлавина во влаге передней камеры (вПК) глаза экспериментальных животных в динамике при стандартном насыщении стромы роговицы растворами для ультрафиолетового (уФ) кросслинкинга в зависимости от площади ее деэпителизации. материал и методы. Исследования проведены последовательно на 42 кроликах. Для насыщения роговицы использовались растворы рибофлавина-0,1% изоосмотический (Ир), Декстралинк (0,1% рибофлавин с 20% декстраном) (Д), 0,1% рибофлавин с 1% гидроксипропилметилцеллюлозой (ГПМЦ) (р) в зависимости от диаметра деэпителизации роговицы: 1-я группа-3 мм (Ир-3, Д-3, р-3), 2-я-6 мм (Ир-6, Д-6, р-6), 3-я-9 мм (Ир-9, Д-9, р-9). После насыщения у всех животных в течение 1 ч с интервалом в 10 мин выполняли забор вПК (252 образца). уровень рибофлавина определяли методом иммуноферментного анализа с использованием тест-системы ID-Vit («Immundiagnostik», Германия). Результаты. установлена стабильная динамика роста уровня рибофлавина в вПК при всех вариантах деэпителизации в течение эксперимента при использовании растворов Д и р с максимумом на 30-40-й минуте. Средство Д во все сроки наблюдения независимо от площади удаленного эпителия роговицы обеспечивало меньшую концентрацию рибофлавина в вПК по сравнению с Ир и р. На 30-й минуте исследования, когда роговица после насыщения растворами считается готовой к уФ-облучению, содержание рибофлавина в вПК составляло от 385±26,1 (Д-9) до 665±28 (р-9) мкг/л. в этих же пределах с 20-й минуты регистрировался уровень фотосенсибилизатора в группах Ир-9, Ир-6, р-6, а также Ир-3, р-3. Достаточная концентрация рибофлавина в вПК или роговице не может гарантировать выполнение качественного и безопасного уФ-сшивания, поскольку площадь удаленного эпителия образует в строме ограниченную зону, пропитанную рибофлавином, так как при выполнении кросслинкинга уФ-воздействию подвергается 8-10 мм диаметра роговицы. Заключение. Безопасный и эффективный стандартный уФ-кросслинкинг роговицы может быть выполнен при оптимальном насыщении стромы рибофлавином преимущественно за счет достаточной площади деэпителизации (не менее 8-9 мм). раствор 0,1% рибофлавина с 1% ГПМЦ обеспечивает быстрое и значимое насыщение стромы фотосенсибилизатором, что позволяет применять его в процедуре уФ-сшивания, в том числе у пациентов с тонкой роговицей.

Цель-проанализировать клинические результаты трансэпителиального кросслинкинга роговичного коллаг... more Цель-проанализировать клинические результаты трансэпителиального кросслинкинга роговичного коллагена по сравнению со стандартным методом при лечении пациентов с кератоконусом. Пациенты и методы. С 2011 по 2013 гг. под наблюдением находились 94 пациента (119 глаз) с кератоконусом I-III степени (по классификации Amsler), из них 43 пациентам (56 глаз) из первой группы, в том числе, 11 пациентам (11 глаз) с исходно тонкой роговицей (380-430 мкм), проведен трансэпителиальный кросслинкинг роговичного коллагена (КРК), 51 пациенту (63 глаза) из второй группы-стандартный КРК. Насыщение роговицы фотосенсибилизатором при трансэпителиальном КРК выполняли посредством электрофореза с использованием аппарата «Поток-1» при силе тока 1 мА в течение 10 минут. Кросслинкинг роговичного коллагена проводили с помощью аппарата «УФалинк» с использованием фотосенсибилизатора «Декстралинк». Результаты. После проведения обоих методов кросслинкинга роговичного коллагена биомикроскопически определялся псевдохейз стромы роговицы: в первой группе в 38 % случаев (30 глаз), который исчезал ко второй-четвертой неделе после КРК, во второй группе-в 100 % случаев (63 глаза), нивелировавшийся в среднем через 3-4 месяца после лечения. По данным оптической когерентной томографии через две недели после трансэпителиального КРК определяли наличие демаркационной линии в 27 глазах (48,2 %), через 1 месяц-в 15 (26,7 %), в дальнейшем демаркационная линия не определялась. После стандартного КРК демаркационная линия присутствовала в течение первых двух недель у всех пациентов (100 %), а сохранялась до 12 месяцев в 2 глазах (3,5 %). Показатель преломляющей силы роговицы через 2 года после трансэпителиального КРК снизился на 1,5 D (р>0,05) по сравнению с исходными значениями; после стандартного КРКна 1,8 D (р>0,05). Некорригированная острота зрения после трансэпителиального КРК через 2 года повысилась с 0,31±0,01 до 0,45±0,05 (р<0,05), после стандартного КРК-с 0,27±0,03 до 0,3±0,05 (р≥0,05). Корригированная острота зрения (КОЗ) в обеих группах имела тенденцию к повышению на протяжении двухлетнего наблюдения. Получение сопоставимых результатов применения трансэпителиального и стандартного методов кросслинкинга роговичного коллагена у пациентов с кератоконусом свидетельствует о предупреждении прогрессирования данного заболевания в 73,2 % и 77,7 % случаев, соответственно, что подтверждает возможность их равноценного использования в клинической практике. Заключение. Трансэпителиальный кросслинкинг роговичного коллагена с насыщением роговицы фотосенсибилизатором методом электрофореза не требует деэпителизации роговицы, хорошо переносится пациентами, не вызывает болевого синдрома. Данная методика КРК практически не уступает стандартному методу кросслинкинга роговичного коллагена, что способствует улучшению оптометрических показателей и стабилизирует течение заболевания.

The aim of the study was to evaluate functional results of Keraring implantation with for keratoconus. Material and methods. Keraring implantation were performed in 46 patients (52 eyes) aged from 16 to 49 years, with progressive keratoconus of the degree I-III of Amsler classification with intolerance of contact correction. To assess the results before and after operation the patients received standard and special methods of research: computer keratotopography («ODPscan», Nidek, Japan), optical coherence tomography («OCT-3», Visante, Germany), corneal confocal microscopy (Heidelberg Retinal Tomographer HRT-III, «Heidelberg Engineering» (Germany) in pre- and post-operative follow-up: 10 days, 1, 3, 6 and 12 months. Results. Uncorrected visual acuity increased from 0.14±0.03 to 0.45±0.15, immediately after the operation. Keratometry value decreased from 54.27±0.91D to 43.61±1.24D. The magnitude of corneal astigmatism decreased from 6.45±1.23D pre-operatively to 4.25±0.96D one month after surgery, with a gradual decrease to 2.91±0.42D and 2.5±0.22D at 6 months and a year after implantation. Conclusions. The intrastromal corneal segments Keraring implantation is a technically simple, effective method of keratoconus treatment, which allows to adjust the associated keratoconic ametropia.
PurposeTo estimate the effectiveness of a single or combined topical neurotrophic factors on dama... more PurposeTo estimate the effectiveness of a single or combined topical neurotrophic factors on damaged retinal ganglion cells (RGCs) in optic nerve crush (OCN) rat model.MethodsThe left ONC was used for achieving RGC degeneration in adult SD rats. Rats were divided into 7 groups with 6 animals in each group, depending on used therapeutic agents which had been administrated by instillations of 100 mm citicoline, 100 mm tauroursodeoxycholic acid (TUDCA), 10 ng/ml neurotrophin‐4 (NT‐4), combined TUDCA/citicoline (Doublet‐1), combined TUDCA/NT‐4 (Doublet‐2), combined TUDCA/citicoline/NT‐4 (Triplet) and PBS. After 2 weeks, the number of remaining RGCs was determined by Brn3a mouse antibody immunostaining of whole‐mount retinas. To examine the regenerative effect, cryosections of optic nerve were made and immunostained for anti ‐200 kD neurofilament heavy and GAP‐43 antibodies.ResultsThe density of RGCs in the PBS group 2 weeks after ONC was significantly less than in the normal control. The density of RGCs in citicoline, TUDCA, Doublet‐1, Doublet‐2, and Triplet groups were significantly higher than that of RGCs in the PBS group. In the Triplet group, the density of RGCs was significantly higher than that in single drug treated eyes. Compared with the PBS group, the decrease in neurofilament immunopositive axon numbers were suppressed significantly in the combination eye drop treatment groups. In the Triplet group, the number of GAP‐43 immunopositive axons was greater than that in the PBS group. Neovascularization in cornea, iris, and retina were observed in 62% of eyes in the Doublet‐1 group.ConclusionsNeuroprotective and regenerative effects can be achieved with topical instillations of combinations of neurotrophic agents. Adverse effects such as neovascularizations had been observed in the Doublet‐1 group. Further studies to identify the mechanisms of neovascularization development is need to be performed.

Aim: To assess prevalence and associated factors of diabetic retinopathy (DR) in a Russian popula... more Aim: To assess prevalence and associated factors of diabetic retinopathy (DR) in a Russian population. Methods: Out of 7328 eligible individuals, the population-based cross-sectional Ural Eye and Medical Study included 5899 (80.5%) individuals aged 40+ years, who underwent a detailed medical and ophthalmological examination. Using ocular fundus photographs and optical coherence tomographic images, we assessed prevalence and degree of DR in 5105 participants. Results: DR was present in 99/5105 individuals (1.9%; 95% confidence interval [CI]: 1.6, 2.3). Its prevalence increased from 6/657 (1.0%; 95% CI: 0.2, 1.6) in the age group of 45-50 years to 24/680 (3.5%; 95% CI: 2.1, 4.9) in the age group of 65-70 years, and decreased to 3/ 153 (2.0%; 95% CI: 0.00, 4.2) in the age group of 80+ years. DR prevalence within the 577 (11.4%; 95% CI: 10.5, 12.2) individuals with diabetes was 99/577 (17.2%; 95% CI: 14.1, 20.2). DR was the cause for moderate-to-severe vision impairment (best corrected visual acuity <6/18 but ≥3/60) in four individuals (4/5105; 0.07%). The purpose of this study is to describe the 26-year-old patient with developed macular hole afte... more The purpose of this study is to describe the 26-year-old patient with developed macular hole after bilateral laser in situ keratomileusis (LASIK). A macular hole with sharp margins and irregular surface of surrounding retina appeared in the left eye of the female 26year-old patient two months after LASIK for correction of myopia (followup of 6 months). Although the best corrected visual acuity (BCVA) after LASIK was 1.0, after the macular hole has developed BCVA became 0.5. After surgery, the final visual acuity recovered to 0.7. Macular hole may develop after LASIK for myopia correction due to unknown changes of vitreoretinal interface. Complete informed consent must be obtained from patients with high myopic eyes before LASIK.

Насыщение роговицы фотосенсибилизатором при трансэпителиальном КРК выполняли посредством электрофореза с использованием аппарата «Поток-1» при силе тока 1 мА в течение 10 минут. Кросслинкинг роговичного коллагена проводили с помощью аппарата «УФалинк» с использованием фотосенсибилизатора «Декстралинк». Результаты. После проведения обоих методов кросслинкинга роговичного коллагена биомикроскопически определялся псевдохейз стромы роговицы: в первой группе в 38 % случаев (30 глаз), который исчезал ко второй-четвертой неделе после КРК, во второй группе-в 100 % случаев (63 глаза), нивелировавшийся в среднем через 3-4 месяца после лечения. По данным оптической когерентной томографии через две недели после трансэпителиального КРК определяли наличие демаркационной линии в 27 глазах (48,2 %), через 1 месяц-в 15 (26,7 %), в дальнейшем демаркационная линия не определялась. После стандартного КРК демаркационная линия присутствовала в течение первых двух недель у всех пациентов (100 %), а сохранялась до 12 месяцев в 2 глазах (3,5 %). Показатель преломляющей силы роговицы через 2 года после трансэпителиального КРК снизился на 1,5 D (р>0,05) по сравнению с исходными значениями; после стандартного КРКна 1,8 D (р>0,05). Некорригированная острота зрения после трансэпителиального КРК через 2 года повысилась с 0,31±0,01 до 0,45±0,05 (р<0,05), после стандартного КРК-с 0,27±0,03 до 0,3±0,05 (р≥0,05). Корригированная острота зрения (КОЗ) в обеих группах имела тенденцию к повышению на протяжении двухлетнего наблюдения. Получение сопоставимых результатов применения трансэпителиального и стандартного методов кросслинкинга роговичного коллагена у пациентов с кератоконусом свидетельствует о предупреждении прогрессирования данного заболевания в 73,2 % и 77,7 % случаев, соответственно, что подтверждает возможность их равноценного использования в клинической практике. Заключение. Трансэпителиальный кросслинкинг роговичного коллагена с насыщением роговицы фотосенсибилизатором методом электрофореза не требует деэпителизации роговицы, хорошо переносится пациентами, не вызывает болевого синдрома. Данная методика КРК практически не уступает стандартному методу кросслинкинга роговичного коллагена, что способствует улучшению оптометрических показателей и стабилизирует течение заболевания.

Methods The population-based Ural Eye and Medical Study included 5,899 (80.5%) individuals out of 7328 eligible individuals aged 40+ years. The aim of the study was to evaluate functional results of Keraring implantation with for keratoconus. Material and methods. Keraring implantation were performed in 46 patients (52 eyes) aged from 16 to 49 years, with progressive keratoconus of the degree I-III of Amsler classification with intolerance of contact correction. To assess the results before and after operation the patients received standard and special methods of research: computer keratotopography («ODPscan», Nidek, Japan), optical coherence tomography («OCT-3», Visante, Germany), corneal confocal microscopy (Heidelberg Retinal Tomographer HRT-III, «Heidelberg Engineering» (Germany) in pre- and post-operative follow-up: 10 days, 1, 3, 6 and 12 months. Results. Uncorrected visual acuity increased from 0.14±0.03 to 0.45±0.15, immediately after the operation. Keratometry value decreased from 54.27±0.91D to 43.61±1.24D. The magnitude of corneal astigmatism decreased from 6.45±1.23D pre-operatively to 4.25±0.96D one month after surgery, with a gradual decrease to 2.91±0.42D and 2.5±0.22D at 6 months and a year after implantation. Conclusions. The intrastromal corneal segments Keraring implantation is a technically simple, effective method of keratoconus treatment, which allows to adjust the associated keratoconic ametropia.
PurposeTo estimate the effectiveness of a single or combined topical neurotrophic factors on dama... more PurposeTo estimate the effectiveness of a single or combined topical neurotrophic factors on damaged retinal ganglion cells (RGCs) in optic nerve crush (OCN) rat model.MethodsThe left ONC was used for achieving RGC degeneration in adult SD rats. Rats were divided into 7 groups with 6 animals in each group, depending on used therapeutic agents which had been administrated by instillations of 100 mm citicoline, 100 mm tauroursodeoxycholic acid (TUDCA), 10 ng/ml neurotrophin‐4 (NT‐4), combined TUDCA/citicoline (Doublet‐1), combined TUDCA/NT‐4 (Doublet‐2), combined TUDCA/citicoline/NT‐4 (Triplet) and PBS. After 2 weeks, the number of remaining RGCs was determined by Brn3a mouse antibody immunostaining of whole‐mount retinas. To examine the regenerative effect, cryosections of optic nerve were made and immunostained for anti ‐200 kD neurofilament heavy and GAP‐43 antibodies.ResultsThe density of RGCs in the PBS group 2 weeks after ONC was significantly less than in the normal control. The density of RGCs in citicoline, TUDCA, Doublet‐1, Doublet‐2, and Triplet groups were significantly higher than that of RGCs in the PBS group. In the Triplet group, the density of RGCs was significantly higher than that in single drug treated eyes. Compared with the PBS group, the decrease in neurofilament immunopositive axon numbers were suppressed significantly in the combination eye drop treatment groups. In the Triplet group, the number of GAP‐43 immunopositive axons was greater than that in the PBS group. Neovascularization in cornea, iris, and retina were observed in 62% of eyes in the Doublet‐1 group.ConclusionsNeuroprotective and regenerative effects can be achieved with topical instillations of combinations of neurotrophic agents. Adverse effects such as neovascularizations had been observed in the Doublet‐1 group. Further studies to identify the mechanisms of neovascularization development is need to be performed.

Aim: To assess prevalence and associated factors of diabetic retinopathy (DR) in a Russian popula... more Aim: To assess prevalence and associated factors of diabetic retinopathy (DR) in a Russian population. Methods: Out of 7328 eligible individuals, the population-based cross-sectional Ural Eye and Medical Study included 5899 (80.5%) individuals aged 40+ years, who underwent a detailed medical and ophthalmological examination. Using ocular fundus photographs and optical coherence tomographic images, we assessed prevalence and degree of DR in 5105 participants. Results: DR was present in 99/5105 individuals (1.9%; 95% confidence interval [CI]: 1.6, 2.3). Its prevalence increased from 6/657 (1.0%; 95% CI: 0.2, 1.6) in the age group of 45-50 years to 24/680 (3.5%; 95% CI: 2.1, 4.9) in the age group of 65-70 years, and decreased to 3/ 153 (2.0%; 95% CI: 0.00, 4.2) in the age group of 80+ years. DR prevalence within the 577 (11.4%; 95% CI: 10.5, 12.2) individuals with diabetes was 99/577 (17.2%; 95% CI: 14.1, 20.2). DR was the cause for moderate-to-severe vision impairment (best corrected visual acuity <6/18 but ≥3/60) in four individuals (4/5105; 0.07%). The purpose of this study is to describe the 26-year-old patient with developed macular hole afte... more The purpose of this study is to describe the 26-year-old patient with developed macular hole after bilateral laser in situ keratomileusis (LASIK). A macular hole with sharp margins and irregular surface of surrounding retina appeared in the left eye of the female 26year-old patient two months after LASIK for correction of myopia (followup of 6 months). Although the best corrected visual acuity (BCVA) after LASIK was 1.0, after the macular hole has developed BCVA became 0.5. After surgery, the final visual acuity recovered to 0.7. Macular hole may develop after LASIK for myopia correction due to unknown changes of vitreoretinal interface. Complete informed consent must be obtained from patients with high myopic eyes before LASIK.

Diabetic retinopathy is known as a neurodegenerative and vascular disease that occurs after some years of poorly controlled diabetes.
