CertiPUR-US^® Annual Registration Renewal and Directory Listing Update | Form C for 2026 Directory

Form C is now only to be used by companies that are currently listed in the directory at certipur.us/directory and are due to renew their registration and confirm or revise their current directory listing(s). Not registered yet? Start here.

How do you know if your company is due to renew for 2026? Unless your directory listing(s) says “valid through 2026,” you are due to renew. Please scroll down to submit a new Form C. This is the only form needed. A completed Form C will ensure your listings are updated for the 2026 directory. If we do not receive an updated Form C each year, your listing(s) will be dropped from the directory and your company will no longer have legal permission to use the trademarked CertiPUR-US^® name and logo.

Before you start this form, please review your directory listing(s) at certipur.us/directory to determine whether they are still accurate.

If there are no changes to your current listing(s), please check the box at the beginning of Section 3; you must still complete, sign and submit the rest of Form C to renew your registration.
If you need to update your current listing(s) or add a brand, please do so in Section 3.
If you need to add or change more than 4 listings, please answer “yes” to the question “I have more than 4 listings of companies/brands and will email [email protected] for a customized approach” in Section 3.

Note: Renewing companies only need to submit Form C. Do not ask your foam supplier to submit a Verification Form A. CertiPUR-US^® administrators will independently verify the supply chain with your suppliers and will contact you if more information is needed.

Indemnify and hold harmless AFPF, Inc. *
