Adenoid hypertrophy is much more common in children than in adults though the condition may persist into adult age and because of its subtle and nonspecific symptoms is often overlooked so that it is easy to be misdiagnosed.
Case Presentation
A 26-year-old male resident of a remote village in Balochistan presented with complaints of nasal obstruction, snoring, and earache, without any complication like epistaxis or cranial nerve involvement. Nasal endoscopy showed nasopharyngeal mass and imaging demonstrated a soft tissue lesion with isodense. The patient underwent an adenoidectomy and the histopathological diagnosis of the excised tissue was of adenoid hypertrophy.
Conclusion
This case illustrates the consideration of adenoid hypertrophy for the differential diagnosis of adult nasal symptoms to achieve proper treatment with symptom resolution

· Increasing urbanization.
· Increasing obesity and thus more people are
involved in such activities.
· Increasing facilities for swimming and water
sports.

Reconstruction of the defect created after total laryngo-pharyngo-esophagectomy has been one of t... more Reconstruction of the defect created after total laryngo-pharyngo-esophagectomy has been one of the challenging task for the head and neck surgeons. There is a lot of debate among the different workers regarding the best method for such reconstruction. Many believe that gastric pull up is still one of the best option in such cases. We are presenting a case of 50 years old lady with extensive squamous cell carcinoma of the hypopharynx which was also involving the larynx and the cervical esophagus. Total laryngo-pharyngo-esophagectomy was done as a primary treatment. For reconstruction of the defect gastric pull up operation was done. Post-operative recovery was uneventful with no major complication. Adjuvant radiation therapy was also given post-operatively. During the follow up period of 6 months, patient was completely alright with almost normal oral feeding and no recurrence of the disease.

Objective: To evaluate the possible risk factors associated with recurrence of stomal recurrence ... more Objective: To evaluate the possible risk factors associated with recurrence of stomal recurrence after total laryngectomy that may be used as evidence of the efficacy of select preventive procedures. Methods: Various clinicopathologic factors of 548 patients who underwent total laryngectomy for laryngeal cancer at our hospital between January 1995 and December 2004 were reviewed. Univariate and multivariate logistic regression models were constructed to evaluate the risk factors for odds ratio (OR) and statistical significance. Results: Recurrence of stomal recurrence was observed in 48 cases and the median time to recurrence was 9.1 months. Univariate analysis showed that primary location (P ϭ 0.000), T-stage (P ϭ 0.000), preoperative tracheotomy (P ϭ 0.001), and positive surgical margin (P ϭ 0.000) were significant factors associated with stomal recurrence. Preoperative tracheotomy had no effect on the incidence of stomal recurrence in the T3 or T4 subgroups. Multivariate analysis showed that T-stage (OR ϭ 1.687; P ϭ 0.049), primary location (OR ϭ 2.387; P ϭ 0.012), and surgical margin (OR ϭ 2.278; P ϭ 0.020) were independent predictive factors for stomal recurrence. Conclusions: The prognosis of patients with stomal recurrence after laryngectomy is very poor. Identification of patients with high-risk factors is essential for offering more selective treatments to prevent their evolution.

There were 46 males and 5 females (mean age 38 years). Fever, cough and haemoptysis were the prime pulmonary complaints while hoarseness, weak voice and episodic dyspnoea were the main laryngeal symptoms. Sites of laryngeal lesions included true vocal cords, arytenoids and false vocal cords. Oedema, pallor, ulcers, vocal cord immobility and thickening were the main laryngeal lesions observed. Twenty-four (47%) cases showed morphological changes in the larynx. The presentation pattern was consistent with the classical description and predilection for laryngeal involvement was not demonstrated.

The mean age was 24 years. More than half of the patients (51%) had subtotal perforations and majority had damage of more than one ossicle. Involvement of all the three ossicles was seen in 30 (40%) cases. The frequency of damaged malleus along with incus was higher than other series. Granulations and subtotal perforations were commoner and could account for it. Extensive cholesteatoma was present in 30 cases.
