  1. Download the Physician Report (template below) to your computer and rename the file to your facility's CCN, facility\u2019s name, and today\u2019s date. Please do not make changes to the format.<\/li>\n
  2. Upload your Physician Acknowledgement Statements along with your Physician Report before you click submit. We accept wet, electronic, and digitally signed Physician Acknowledgement Statements.<\/li>\n
  3. You will receive further instruction from Acentra Health if deficiencies are found.<\/li>\n<\/ol>\n
